Objective(s):This subject aims to research the factor risk of death of elderly ischemic stroke patients by collecting elderly cases of medical records of yan 'an hospital affiliated to Kunming medical university on January 1,2016-December 31,2017.The clinical data and biochemical examination of the brain infarction cases were retrospectively analyzed.This subject engaged in the elderly brain infarction to provide disease index statistics and analysis of factors related to death for clinical doctors that is more convenient for clinical workers by making effective preventive measures,improving the treatment efficiency,reducing the fatality rate in elderly patients with cerebral infarction.Methods:Collection of elderly cases of brain infarction clinical medical records on January 1,2016-December 31,2017 of Yan 'an hospital affiliated to Kunming medical university.1487 cases of senile cerebral infarction patients in hospital were retrospectively analyzed data,according to the patient's gender,age,illness,end group.Collect and record the patient's past and personal history,complications after admission,blood pressure,temperature,NIHSS score,and the first biochemical test data.SPSS 23.0 software was used for data processing,and the measurement data were statistically described by means of mean plus or minus standard deviation,and the counting data were represented by frequency.Single factor analysis of variance or independent sample t test were used for the comparison between groups satisfying normal state and homogeneity of variance.The counting data were examined by X2 test.First of all the related factors of death of elderly cerebral infarction was screened out,then there might be significance of the independent variable is introduced into the multiple factors analysis,the related parameters of the multi-factor logistic regression analysis and the correlation of senile cerebral infarction,it is concluded that the death of elderly cerebral infarction was related risk factors.Results:1.A total of 1,487 elderly hospitalized patients with cerebral infarction were included in this retrospective study.2.Gender and age distribution statistics:in 1487 cases of senile cerebral infarction,the ratio of male to female is close to 1.29:1.There were 868 young-aged people,accounting for 58.37%,598 middle-aged and old people,accounting for 40.22%,and 21 old-aged people,accounting for 1.41%.The results showed that the young-aged group aged 60 to 74 was the main component of the total number of cerebral infarction in the elderly,followed by the middle-aged group and the old-aged group.The ratio of male to female in the young-old group was 1.3:1,that of the middle-aged group was 1.29:1,and that of the old-aged group was 0.91:1.3.Mortality statistics of all age groups and gender groups:there was no significant difference in the gender distribution of elderly hospitalized patients with cerebral infarction in kunming area;With the increase of patients' age,the mortality rate of cerebral infarction in the elderly increases gradually.4.The susceptible factors and complications in the susceptible factors:a history of high blood pressure patients accounted for 68%,followed by after drinking history,diabetes,smoking history,history of stroke,coronary heart disease,atrial fibrillation.In the comparison between the gender groups,age groups and the returnees,more men than women had a history of drinking and stroke,and the difference was statistically significant.The differences of hypertension,diabetes,smoking,alcohol consumption,stroke,coronary heart disease and atrial fibrillation among all age groups were statistically significant.Compared with the survival and death groups,the mortality groups had more patients with atrial fibrillation than the survival groups,and the difference was statistically significant.The most common complication in all cases is arrhythmia.The incidence of arrhythmias complicated by cerebral infarction in the elderly was different between the sex group and the age group.The number of elderly patients who died in the hospital of cerebral infarction complicated with arrhythmia,pulmonary infection,postinfarction hemorrhage,epilepsy and upper digestive tract hemorrhage was higher than that of the survival group,all of which had statistical significance.The type of arrhythmias in the elderly patients with cerebral infarction was the most common,with atrial fibrillation accounting for 65%of patients with arrhythmias.5.The elderly patients with cerebral infarction in Kunming region,the fasting blood glucose value,peripheral blood leukocyte and serum creatinine for the first time after admission and NIHSS evaluation values significantly higher than the survival group,the difference was statistically significant.6.Multiariable logistic regression analysis showed that age group(OR = 1,212,95%CI 1.083 to 1.357,P<0.01),and pulmonary infection(OR = 22.178,95%CI 2.463 to 199.729,P<0.01),peripheral blood leukocyte(OR = 1.609,95%CI 1.236 to 2.094,P<0.01)were significantly correlated with death of cerebral infarction.For the elderly cerebral infarction they were the independent risk factors of death.Conclusion(s):1.The young-aged group which aged 60-74 are the main components of the elderly patients with cerebral infarction in Kunming area.There was no significant difference in the sex distribution of the elderly patients with cerebral infarction.With the increase of patients' age,the mortality rate of cerebral infarction in the elderly increases gradually.2.Over 60%of the elderly patients with cerebral infarction have a history of hypertension in Kunming;Arrhythmias occurred in 18.83 per cent and lung infections in 14.93 per cent of the population.We should strengthen the control of blood pressure in clinical work.Hospital of cerebral infarction patients need strengthen ECG monitoring,prevention and treatment of arrhythmia.Prevention and treatment of pulmonary infection should be taken timely and effective treatment measures.3.Age,pulmonary infection and peripheral leukocyte count are independent risk factors for in-hospital death of senile cerebral infarction.Serum creatinine,fasting glucose,NIHSS score values,upper gastrointestinal bleeding,bleeding again after cerebral infarction,arrhythmia,epilepsy and a history of atrial fibrillation may be associated with the death of senile cerebral infarction.
