To Explore The Relationship Between Helicobacter Pylori Infection And Serum PCT,IL-6,hs-CRP Levels

Abstract/Summary:
Objective:The inflammatory and immune responses are important pathogenic mechanisms of Helicobacter pylori,in which IL-6 and hs-CRP may be important cytokines and inflammatory mediators involved in the pathogenesis.PCT can well reflect the active degree of inflammatory response.In this study,the levels of serum PCT,IL-6,and hs-CRP levels in patients with HP-positive chronic gastritis and HP-negative chronic gastritis were measured to determine the relationship between the three factors and Helicobacter pylori(HP)infection,as well as their role in HP-related diseases.Methods:We retrospectively selected 58 patients with chronic gastritis who were admitted to the Department of Digestion,Second Affiliated Hospital of Kunming Medical University from May 2016 to December 2017,including 21 males and 37 females,average age 56.33±15.83 years.The results of gastroscopy,barium meal,13C urea breathing test(UBT),serum PCT,IL-6,hs-CRP,cytotoxin-associated protein A(CagA)antibody were collected.According to the results of 13C-UBT and CagA antibodies,patients with chronic gastritis were divided into four groups:HP+ group(group A),HP-group(group B),HP+/CagA+ group(group C)and HP+/CagA-group(Group D).Serum inflammatory factor levels were compared among these groups.Results:1.The serum PCT level in the HP+ group(group A)was 0.071(0.050,0.082)ng/ml,and 0.034(0.021,0.055)ng/ml in the HP-group(group B).HP+ group was higher than HP-group(P<0.05).The plasma EL-6 levels in HP+ group and HP-group were 3.04(1.50,6.22)pg/ml and 1.50(1.50,3.59)pg/ml,respectively.HP+ group was higher than HP-group(P<0.05).The plasma hs-CRP levels in group A and group B were 3.25(1.04,5.80)mg/L and 2.89(0.76,4.87)mg/L,respectively.The hs-CRP level was similar between two groups(P>0.05).2.The level of serum PCT in the HP+/CagA+ group(group C)was 0.077(0.055,0.085)ng/ml,and 0.064(0.030,0.079)ng/ml in the HP+/CagA-group(group D).The PCT level was similar between two groups(P>0.05).Serum IL-6 levels in group C and D were 4.02(2.06,12.70)pg/ml and 2.23(1.50,3.24)pg/ml,respectively.The HP+/CagA+group was higher than HP+/CagA-group(P<0.05).The plasma hs-CRP levels in groups C and D were 4.72(2.91,6.74)mg/L and 1.44(0.55,4.30)mg/L,respectively.The HP+/CagA+ group was higher than HP+/CagA-group(P<0.05).3.Serum inflammatory factor levels were compared among the HP+/CagA+ group,HP+/CagA-group and HP-group.The plasma PCT levels in HP+/CagA+ group and HP+/CagA-group were both higher than those in HP-group(P<0.05).The plasma IL-6 and hs-CRP levels in HP+/CagA+ group were higher than those in HP-group(P<0.05),and were similar between HP+/CagA-group and HP-group(P>0.05).Conclusions:1.HP infection(either CagA+ strain or CagA-strain)can cause elevated levels of serum PCT.and it’s expression level was not related to the CagA virulence factor.2.The serum IL-6 and hs-CRP levels were elevated in patients with CagA+ strain,and similar between HP+/CagA-group and HP-group.Their expression levels were related to the CagA virulence factor.Indicating that the CagA+ strain with stronger pathogenicity caused more severe inflammation.
