Objective:The clinical data of patients with severe thoracolumbar burst fracture in the single segment treated in our department between October 2015 and June 2017 were collected.The posterior approach vertebral corpectomy and vertebral canal decompression and titanium mesh bone grating and pedicle screw fixation and short-segmental fusion were performed.After the internal fixation treatment,the patient's recovery and related influencing factors were analyzed to guide the standardized operation of the patients.Methods:Data of the preoperative and postoperative ASIA functional grading,VAS scoring,Oswestry dysfunction index,and postoperative bone grafting vertebral body's healing status and clinical imaging related parameters measured(such as the height of anterior,midline and posterior vertebrae at the level of the fracture,spinal canal occupancy rate,Cobb's angle,etc.)were collected.Statistical analysis of the data obtained by SPSS 19.0 software.Results:In this study,there were 21 males and 11 females,with an average age of 41.03± 12.16 years old(29-53 years old).The mechanisms of injury were traffic accident in 12 cases,accounting for 37.5%,and 16 patients were falling down injury,accounting for 50.0%,bruise in 4 cases,accounting for 12.5%.Patient's gender,age,Denis classification,the difference was not statistically significant ASIA grade,VAS score,ODI score,Cobb's angle,the height of anterior?midline?posterior vertebrae before and after the operation,spinal occupancy ratio,and the ratio of bone healing state,there was no significant difference between the patients,p>0.05.After the operation,the average height of the vertebrae at the level of fractures was restored and the pain feeling were relieved.Conclusion:According to fracture morphology and imaging parameters,patients with severe thoracolumbar burst fractures(Denis Burst fractures of type D and E)were performed the posterior approach vertebral corpectomy and vertebral canal decompression and titanium mesh bone grating and pedicle screw fixation and short-segmental fusion.The related factors affecting postoperative recovery are preoperative VAS value,spinal occupancy ratio,the height of midline vertebrae 3-day and 6-month after the operation.
