Objective: Two-dimensional speckle tracking imaging(2D-STI)is used to evaluate the early changes of left and right ventricular systolic function in patients with uremia.Through the study of clinical data of patients with uremia and uremia with diabetes mellitus,which could help clinicians take interventions actively to prevent further heart function from deteriorating as early as possible.Methods: A total of 58 uremic patients participated in this study.We divided the patients into two groups(diabetic or non-diabetic patients),32 patients were diabetic patients,and 26 patients were non-diabetic patients.Control group included 20 healthy individuals in the study.Obtained and saved the two-dimensional animation video of the left ventricular long axis view,apical four-chamber view,apical two-chamber and three short-axis views.2D-STI technique was used to measure the longitudinal systolic strain,short-axis strain(strain rate)of left ventricular,and the peak systolic strain of the global right ventricular wall(G-PLSS),then compared the differences among the groups.Results: 1.It was no significant difference in age,sex and body mass index(BMI)among the three groups(P>0.05).2.There was no significant difference among the three groups in the LVEF and LVIDd(P>0.05).It was significant difference in IVSTd and LVPWd among the three groups(P<0.05),but also no significant difference between uremia group and the uremia with diabetes mellitus group(P>0.05).3.The systolic longitudinal strain reduction of left ventricular apical three sections between patients with two uremia groups and the control group were statistically significant(P<0.05).There was still significant difference(P<0.05)in the reduction of systolic longitudinal strain between the group of uremia patients with diabetes mellitus and the non-diabetic group.The systolic circumferential strain reduction of left ventricular three sections between patients with two uremia groups and the control group were statistically significant(P<0.05).There was significant difference between the two groups in the reduction of systolic circumferential strain in patients with uremia with diabetes mellitus and control group(P<0.05).But there was no significant difference between non-diabetic uremia group and control group(P>0.05).4.There was no significant difference in RVDd,TAPSE and LVEF among the three groups(P>0.05)of routine parameters.It was significant difference in IVSTd among the three groups(P<0.05),but there was no significant difference between non-diabetic group and the uremia with diabetes mellitus group(P>0.05).5.G-PLSS in uremic patients with and without diabetes mellitus group were lower than those in control group(P<0.05),and uremic patients with diabetes mellitus compared with the non-diabetic uremia group,the reduction was more significant,also between the two groups in the right ventricular strain parameters was significant difference(P<0.05).Conclusion: It can be concluded that the left and right ventricular function is impaired in uremia patients whether or not with diabetes mellitus,Two-dimensional speckle tracking imaging can detect the early changes of ventricular systolic function,and myocardial damage in uremia patients with diabetes mellitus is more serious.Controlling blood glucose can benefit uremic patients.
