Objective: Using real time three-dimensional echocardiography(RT-3DE)and two dimensional speckle tracking imaging(2D-STI)to measure the paraments of left ventricular function in patients with type 2 Diabetes mellitus(2-DM),to discuss the2D-STI and RT-3DE technology in the evaluation of left ventricular function in patients with type 2 diabetes mellitus valueMethods: Normal two dimensionnal echocardiography(2-DE)?2D-STI and RT-3DE was performed on 32 patients with 2-DM(group DM)and 32 healthy subjects(control group).using conventional two-dimensl gray scale imaging to acquire Apical four-chamber ? two-chamber and LV long-axis view images.Opening a CMQ to measure longitudinal segment strain(Ls S)? longitudinal segment strain rate E(Ls SRe)?longitudinal segment strain rate A(Ls SRa)and segment the time to peak systolic strain(Ts).Calculating mean as longitudinal global strain(Lg S)?longitudinal global strain rate E(Lg SRe)?longitudinal global strain rate A(Lg SRa)?global the time to peak systolic strain(Tg).Aquiring Full Volume on four-chamber,open the3 DQa software measurement,sequence analysis,the software automatically calculates the left ventricular end diastolic volume(EDV),end systolic volume(ESV),left ventricular ejection fraction(LVEF),generating 16-segement volume time curve,and calculating curve of left ventricular systolic volume reaches the minimum time difference of the standard deviation correction value(Tmsv-16-SD%)?the maximum time difference(Tmsv-16-Dif).All parameters were analyzed by SPSS 21 statistical software,and the sex ratio was tested by chi square test.The data of the two groups were compared with the t test of two independent samples,the relationship between the two parameters: Person correlation analysis.Results:1.Comparison left ventricular diastolic funtion Compared with control group,The Ls SRe?Lg SRe of DM group was lower(P<0.05),There was no statistical difference between two group of Ls SRa?Lg SRa(P>0.05),Lg SRe/Lg SRa in DM group was lower and value was less than 1,E/Lg SRe was increased(P<0.05).2.The comprative of left ventricular systolic funtion Compared with control group,Ls S?Lg S was decreased in the DM group(P<0.05).,The peak systolic strain peak time of DM group was longer(P<0.05).EDV?ESV was increased in DM group,and LVEF was lower(P<0.05).The regional and global myocardial strain were correlated with LVEF.The correlation of RT-3DE were higher than 2DE(r=0.839,p<0.01;r=0.464,p<0.01)3.The comprative of left ventricular synchronism The 16-segmental volume curves of control group was oederly arrangment,in contrast DM group was disorderly,Tmsv-16-SD%?Tmsv-16-Dif of DM group was higher than control group(P<0.05).Conclusion:Real time three dimensional echocardiography and two-dimensional speckle tracking imaging can evaluate left ventricular function and synchrony in patients with type 2 diabetes mellitus,and can early detection of diabetic myocardial damage.
