Objective To explore early right ventricular longitudinal function in patients with systemiclupus erythematosus and to explore its clinical value.Methods Fouty-one patients with SLE(SLE group) and thirty-three healthy controls(control group) were enrolled. Then the tricuspid annular plane systolic excursion(TAPSE),peak systolic velocity(PSv),isovolumic contraction peak velocity at the tricuspid annulus(IVCv) were measured with tissue Doppler imaging. longitudinal peak systolic strain of the basal,mid,apical segments of free wall and strain rate(Ss, SRs,SRe, SRa)of right ventricular was measured with two-dimensional speckle tracking imaging,and Global systolic strain and global strain rate(GSs) was calculated. according to the results of clinical symptoms and laboratory examination to evaluated the disease activity score(SLEDAI) of patients with SLE. Every parameter was compared, and corellaration among the strain of the whole right ventricular 、duration time and SLEDAI scoreswere analyzed. Results Compared with control group,conventional parameters RVWT、RVOT distal、RVD、FAC、LVEF 、E’、A’、E’\ A’、E \ E’,IVRT are not significantly different; Compared with control group,TAPSE、PSv、IVCv decreased(P<0.01); Compared with control group, Ss in basal and middle segments decreased in SLE group(P<0.01),Ss in apical segment are not different(P>0.05); SRs、SRa in basal segments decreased in SLE group(P<0.01),while SRe in basal segment and SRs、SRa、SRe are not significantly different(P>0.05);GSs、GSRs、GSRa significantly decreased in SLE group(P<0.01); Among SLE,there were good correlaration of TAPSE、PSv、IVCv with GSs;there were good correlaration of disease during 、SLEDAI values with GSs、GSRs、GSRa(P <0.01); Conclusion right ventricular systolic and diastolic dysfunction may be found in SLE patients without clinical symptom of heartfail,2D-STI could accurately assess right ventricular systolic and diastolic function in SLE patients.
