Objective:By reviewing and collecting clinical data of patients with ischemic colitis,the differences between the elderly and young groups in disease characteristics were analyzed,and explored the possible risk factors associated with the pathogenesis of ischemic colitis,raised awareness of the disease for early diagnosis and improvement of the prognosis.Methods:1.In this study,169 patients who were diagnosed with IC from January 2007 to December 2017 in the First Hospital of Jilin University were selected as the research objects,and completed the collection of clinical data.According to age,the objects were divided into the elderly group(?60 years old)and the young group(<60 years old),including 100 elderly patients and 69 young patients.The two groups were compared with the general conditions(gender,age,underlying disease,lifestyle),clinical features(clinical performance,laboratory tests),colonoscopy(lesion location,classification,pathology),treatment and prognosis.Statistical analysis used SPSS17.0statistical software.Chi-square test,Fisher's exact test,t-test(approximate t test)or non-parametric test were used to compare the indicators between the two groups.2.From January 2007 to December 2017,169 patients who were diagnosed as IC in the First Hospital of Jilin University were selected as the case group.Simultaneously,169 patients who underwent colonoscopy and/or pathological examination to exclude ischemic colitis were selected as the control group.Screening for possible risk factors associated with the onset of ischemic colitis was performed by univariate and multivariate analysis.Univariate analysis was performed using chi-square test or t-test.Multivariate analysis was performed using unconditional two-class logistic regression analysis.Results:1.The incidence of IC in the young and elderly groups was dominated by women,and there was no statistical difference in morbidity of men and women between the two groups(P>0.05).The rate of IC in the elderly group combined with hypertension,coronary heart disease,diabetes,hyperlipidemia,and respiratory diseases was significantly higher than that in the young group,and there was statistical difference(P<0.05).In the elderly group and the young group,IC patients had hematochezia,abdominal pain and diarrhea as the main clinical manifestations,and there was no statistical difference(P>0.05).The WBC and BUN levels in elderly patients with IC were higher than those in young patients,while ALB and HGB levels were lower than those in young patients,and there was statistical difference(P<0.05).The sigmoid colon and descending colon were the main lesions in the elderly and young groups.The descending colon was the most common lesion,followed by the sigmoid colon,and there was no statistical difference between the two groups(P>0.05).According to the Marston classification,the type of colonoscopy was mainly transient type in the elderly group and young group,followed by structuring type and gangrene type,and there was no statistical difference(P>0.05).The pathological difference between the two groups was not statistically significant(P>0.05),and mucosal inflammation was the main factor.The number of days of hospitalization in the elderly group with IC was longer than that in the young groups,and there was statistical difference(P<0.05).In the elderly group,98 cases were given with medical treatment and 2 cases were treated with surgical treatment,while in the young group68 cases were given with medical treatment,and 1 case was given with surgical treatment,and there was no significant difference between the two groups(P>0.05).The condition of the patients in both groups was improved and then discharged,and there was no statistical difference(P>0.05).2.Univariate analysis showed that the proportion of hypertension,diabetes,coronary heart disease,hyperlipidemia,cerebrovascular disease,abdominal surgery,and constipation was significantly higher in the case group than that in the controlgroup,and there was statistical difference(P<0.05).There was no significant difference between the two groups in respiratory disease,kidney disease,atrial fibrillation,smoking and drinking(P>0.05).Multivariate analysis showed that hypertension,diabetes,hyperlipidemia,and abdominal surgery history were risk factors for IC(P<0.05).Conclusion:1.The proportion of females was higher than that of males in both the elderly group and young group.Compared with the young group,elderly group with IC were more likely to have hypertension,coronary heart disease,diabetes,hyperlipidemia,and respiratory diseases.The most common clinical manifestation of IC was hematochezia,followed by abdominal pain and diarrhea.The endoscopic features of elderly patients with IC were similar to those of young people,and the main pathology was mucosal inflammation,the descending colon and sigmoid colon were the major lesions,and the main lesion type was transient type.The elderly and young patients with IC were mainly treated with internal medicine while the elderly patients have longer hospital stay and there was no difference in prognosis.2.Hypertension,diabetes,hyperlipidemia,and abdominal surgery history were risk factors for IC.
