The CHADS2 Versus CHA2DS2-VASc Scores For Predicting Ischemic Stroke Among Chinanese Patients With Atrial Fibrillation: A Meta-analysis

Objective Chads2 score,Cha2ds2-VASc score is clinically used in nonvalvular atrial fibrillation(non-valvular atrial fibrillation,NVAF)in patients with ischemic stroke risk assessment scale,the size of the primary prevention of atrial fibrillation after ischemic stroke plan provides the main foundation,obtained the ACC/AHA/ESC af management guidelines recommend [1].For China in patients with atrial fibrillation,two kinds of evaluation which is more suitable for clinical guidance of anticoagulant,consult literature at home and abroad,the system have not been reported.Based on this study on Chinese patients with atrial fibrillation in the Chads2 score,Cha2ds2-VASc score under various risk groups in the incidence of ischemic stroke Meta analysis,in order to prevention of ischemic cerebral stroke patients with atrial fibrillation in China to provide clinical evidence of evidence-based medicine.This study by capping keywords retrieval,and application of computer retrieval China academic journal database(CNKI),D Pu Zhong Wen journals database,wanfang database,Pubmed,Medline and the Cochrane Library,Embase database,such as collection at home and abroad since January 2010 to December 2017,the Chinese Chads2 score in patients with atrial fibrillation and Cha2ds2-VASc grading research literature in both English and Chinese,on the basis of established and exclusion criteria,with 984 references,preliminary screening to meet the requirements by document management tools,read the title,abstract and full text,further ruled out is not in conformity with the conditions of the literature,eventually into 5 references in English and Chinese with 3 references,to extract the relevant data,the application of Revman5.3 Meta analysis software,calculate the relative risk(RR)and 95% confidence interval(95% CI).System evaluation Chads2 score,Cha2ds2-VASc score low,medium and high risk in patients with atrial fibrillation group of predictive value of the risk of ischemic stroke.MethodsResults Cha2ds2-VASc score of valvular atrial fibrillation patients low-risk groups in China,the predictive value of stroke risk in a Chads2 score is high,the high risk group,Cha2ds2-VASc score with Chads2 score the predictive value of no difference(P = 0.06).Statistical analysis results for low-risk groups: RR = 1.53,95% CI(1.41,1.41),Z test results for P < 0.00001;Moderate groups: RR = 1.15,95% CI(1.10,1.10),Z test results for P < 0.00001;High-risk groups: RR = 1.10,95% CI(1.00,1.00),Z test results for P = 0.06 > 0.05.Conclusion According to this research into eight clinical research system after analysis the following conclusions: Cha2ds2-VASc score in predicting China nonvalvular atrial fibrillation low-risk group,the moderate group superior value to the risk of stroke in patients with Chads2 score,able to identify real low risk patients;In the high-risk group of patients the predictive value of no significant difference.
