Object:To analyze the clinical features associated with steatosis and fibrosis in patients with chronic hepatitis C.Material and Method:Two hundred and eighteen patients were enrolled.Which were all infected with HCV,the patients were all from the Hepatology Department of the first hospital of Jilin university.Physical examination,body mass index,liver function tests,HCV-RNA and HCV genotype,complete blood count and abdominal ultrasonography were performed.And then we calculated the NLR,APRI.Firstly,patients were assigned into two groups A and B,patients from group B all clearly diagnosed with fatty liver,while group A were didn't,we analyzed all the laboratory tests,BMI,APRIand then compared these factors between the two groups.We used SPSS20.0,Independent-samples T test was used to analyze data according with normal distribution,non-parametric test was used to analyze data that didn't complied with normal distribution.We selected Logistic regression to analyze the risk factors of fatty liver in patients with chronic hepatitis C.Result:There was no statistical difference in the age and gender between the two groups of patients,indicating that the baseline of the two groups of patients was basically the same,The patient's minimum age was 17 years old,the maximum age was 76 years old,the median age was 53 years old,the age of non-fatty liver group was 54(45.5-61)years old,and the fatty liver group was 53(47-57)years old.In the study,there were121 male patients,accounting for 55.8%,and 96 female patients,accounting for44.2%.There was no difference in gender between the two groups.The virological methods were the same for the two groups of patients.there was no genotype difference between the two groups.In this study,there was no difference in the incidence of diabetes,hypertension,and smoking history between the two groups.Comparison of blood lipids between the two groups: The results suggest that the levels of triglyceride and low-density lipoprotein cholesterol in the fatty liver group were higher than those in the non-fatty liver group.The statistical difference between the two groups was significantly(Z=-3.568 and P=0.000),and Z=-2.518,P=0.012.The liver function of the two groups was compared,including AST,ALT,GGT,ALP,ALB and TBIL.There was no statistically significant difference between the two groups.Compared with the two groups,the NLR in the group without fatty liver was higher,and the difference was statistically significant.There was no statistical difference in platelets.The BMI was normally distributed between the two groups,with a BMI of23.33±0.24 in the non-fat liver group and a 25.25±0.34 in the fatty liver group.The BMI level in the fatty liver group were higher than those in the fatty liver group.(t=-4.209 P=0.000).Comparing coagulation function between the two groups,the median PT fatty liver group was 10.9 s,non-fatty liver group was 11.5 s,and fatty liver group was significantly lower than non-fatty liver group(Z=-2.883 P=0.004).The INR fatty liver group was 0.94,the non-fatty liver group was 0.98,and the fatty liver group was lower.There was a statistically significant difference between the two groups(Z=-2.190 P=0.029).The APRI scores of the two groups were no statistical difference(2=0.078,P=0.781).Logistic regression analysis showed that body mass index(OR: 1.314,95% CI: 1.150-1.501)was an independent risk factor for fatty liver disease in patients with CHC.The level of NLR and PT in patients with fatty liver were lower than those in non-fatty liver.NLR(OR: 0.357,95% CI: 0.185-0.685),PT(OR: 0.572,95% CI: 0.353-0.929).Conclusion:The patient's minimum age was 17 years,the maximum age was 76 years,themedian age was 53 years,the age of non-fatty liver group was 54(45.5-61)years,and the fatty liver group was 53(47-57)years old.In the study,there were 121 male patients,accounting for 55.8%,and 96 female patients,accounting for 44.2%.Body mass index(OR: 1.314,95% CI: 1.150-1.501)is an independent risk factor for fatty liver disease in patients with CHC.
