Objective: To compare the clinical efficacy of percutaneous nephrolithotomy lithotomy and ureteral soft lithotripsy in the treatment of complex nephrolithiasis.Methods: A retrospective analysis of 171 patients with complex renal stones who were hospitalized between September 2013 and July 2017 was performed.Sixty patients underwent ureteral lithotripsy and 111 patients underwent percutaneous nephrolithotomy.RESULTS: Percutaneous nephrolithotomy(PNL): operation time: 35-91 min,mean operation time: 54 min,amount of blood loss: hemoglobin loss was 6-20 g/L,mean value was 11 g/L,stone clearance rate : The rate of one-time stone clearance was 88.3%(98/111).Postoperative complications: The overall incidence was 25.2%(28/111).The number of days after discharge: 3-12 days,and the average length of stay in hospital was 6 days.Ureteral soft lens group(URL): operation time: 40-155 min,average operation time 87 min,surgical bleeding volume: 0-6g/L,average 3g/L.Stone clearance rate: The rate of stone clearance in one period was 76.7%(46/60).Postoperative complications: The overall incidence was 8.3%(5/60).The number of days after discharge: 2-6 days.The average length of hospital stay was 3 days.Conclusion: We believe that percutaneous nephrolithotomy and ureteral soft lens have definite curative effect in treating complex kidney stones,but all have their own advantages and disadvantages.When selecting surgical methods,according to the patient's specific conditions,the surgeon's grasp of the technology The situation,stone size,structure,and location are all taken into consideration to select the best surgical approach.The main drawbacks of this study are the small sample size and single-center study.There is no stratified study and there is no long-term follow-up data.Therefore,there are inevitable biases.Whether or not it has clinical application and promotion value requires a large sample of multi-center studies to confirm.
