Study On Burden And Influencing Factors For Primary Caregivers Of Patients With Advanced Cancer

Objective:1.To understand the reliability and validity of Chinese version of terminal cancer care load scale;2.To understand the care burden of caregivers of advanced cancer patients in Xinjiang area;3.To understand the social support status of the main caregivers of patients with advanced cancer;4.To compare the difference of family cohesion and adaptability between the two groups of advanced cancer patients with different degree of care load;5.To explore the correlation between caregiver burden and family cohesion,adaptability and social support in caregivers of patients with advanced cancer;6.To explore the factors influencing caregiver burden of caregivers of patients with advanced cancer in Xinjiang.Methods:The descriptive approach used in this study,from May 2016 to August 2017,Urumqi City Hospital Department of some three grade tumor hospital,the main hospital to accept the treatment of late stage cancer patients and family caregivers,a total of 220 cases.Methods of questionnaire survey,the caregiver demographic data,terminal cancer caregiver burden scale,social support scale and family adaptability and Cohesion Scale,respectively,to take care of advanced cancer burden,social support and family cohesion and adaptability of the investigation,and to evaluate the reliability and validity of load of care table;in accordance with the burden of care scores,number of survey will be divided into high and low two groups,compared to take care of family cohesion and adaptability,and analyze the relationship between the caregiver burden and the scores of cohesion and adaptability score.Statistical method,using EpiData3.1 for data input,using SPSS 17 for statistical analysis.Count data with number,percentage of description,measurement data using mean and standard deviation description;statistical methods descriptive statistics,two independent samples t test,single factor analysis,ANOVA analysis,and spearman correlation analysis,the influence factors of caregiver burden multivariate analysis using multiple linear regression analysis,P<0.05 had statistical significance.Results:1.The 17 items of the Chinese version of cancer terminalcare load scale Cronbach'?coefficient 0.901,the Cronbach' coefficient of each dimension was 0.829-0.912,the split half coefficient was 0.874,and the test-retest reliability was0.792.The mean of content validity was 0.916;after factor analysis,the cumulative contribution rate of 4 common factors was 61.247%,and the load of each item factor was >0.5.2.Patients with advanced cancer caregivers of the total average load(52.56 +10.38),which,according to the average scores,from high to low: economic load(3.21 +0.92),physical workload(3.19 + 0.71),live load(3.13 + 1.08)and spiritual load(2.80.0.47),according to the degree of care burden followed by the size of economic load,physical and mental load,load and load of spiritual life.3.The total score of social support was(31.08 + 7.15),objective support(7.52 + 2.06),subjective support(19.45 + 7.19),and support utilization(5.02 + 1.69).The total score of social support was lower than the norm value in China,the difference between the two was statistically significant(P<0.01).4.The familiarity and adaptability of two groups of families with high and low load levels were compared.The scores of families with high levels of care of high load groups,the degree of intimacy dissatisfaction and the degree of adaptive dissatisfaction were higher than those in the lower scores,and the difference was statistically significant(P<0.01),actual family cohesion,adaptability scores were lower than the low group,the difference between the two was statistically significant(P<0.01).5.The correlation between caregiving burden and family cohesion,adaptability and social support.Take care of the total load and the actual intimacy(r =-0.358,P<0.001),the actual adaptability(r=-0.215,P=0.001),the degree of dissatisfaction of intimacy(r=0.389,P=0.001),the degree of adaptive dissatisfaction(r=0.381,P=0.001)and Subjective support(r=-0.265,P=0.001)are related with statistical significance;Primary care there is a negative correlation between the total score of social support and the load(r=0.425,P<0.05).6.Multivariate analysis of caregiver burden,multivariate linear regression analysis showed that the effect of care income,economic pressure,family income,the total time to take care of patients,self-care,adaptability and intimacy,adaptability,intimacy satisfaction satisfaction and subjective support for the factors affecting the care burden(P<0.05).Conclusion:Chinese version of terminal cancer caregiver burden scale has good reliability and validity,suitable for use in Xingjiang region of China;Xinjiang area of advanced cancer patients to take care of the load degree is not high and moderate level;and caregiver burden was negatively correlated with the level of social support;actual family cohesion and adaptability of the actual situation is not optimistic,and with care there is a negative correlation between the load,need to give targeted social support,in order to improve thepatient's family function and family relationship,reduce caregiver burden of care,improve the quality of life of patients and caregivers.
