| Purpose:To understand the prevalence of overweight and obesity among pupils in Dalian,to master the obesity knowledge,attitude and health behaviors among pupils and their parents,and to evaluate the effect of health education intervention on the prevention and control of childhood obesity based on the theory of "knowledge and belief" and self-efficacy theory.Promote children’s healthy growth,provide reference for comprehensively formulating and implementing targeted group intervention strategies,explore the establishment of "health promoting schools" model,and provide academic basis for carrying out practicable school intervention policies.Methods: A cluster random sampling method was used to randomly select two elementary schools with similar scale and curriculum settings in Xigang District of Dalian.All students in grade 3-5 and their parents were selected as research subjects.Half of the each grade students were in the intervention group for health education,and the remaining classes were control groups without any intervention.At the baseline and final stages,medical examinations were conducted for all students by professionals.The physical examinations included height,weight,waist circumference,blood pressure,body fat percentage,body water volume,muscle mass,basal metabolic rate,etc.Students and parents completed the students and the parent version of the "Primary School Health Questionnaire" to evaluate interventions impact on childhood obesity-related knowledge,attitude,behavior and self-efficacy,to prevent and control of childhood obesity.Questionnaires and physical examination data were recovered,after the logic check,finished the establishment of a database with Epidata3.1 and double data entry,using SPSS 19.0 for statistical analysis.Results:1.Detection of overweight and obesity among pupils in Dalian: The detection rate of overweight among pupils in Dalian was 13.2%,the obesity was 20.7%;the detection rate of overweight and obesity was higher in boys than girls;the students in urban areas were obese than that of rural students.2.Cognition of schoolchildren and their parents about body weight: The consistency between the self-assessment of body size and the actual BMI was poor,the consistency coefficient of the intervention group was 0.357,and the control group was0.388.The parents’ actual assessment was moderate,the consistency coefficient of the intervention group was 0.476,and the control group was 0.471.3.Obesity-related knowledge,attitude,behavior and self-efficacy of pupils in Dalian: Obesity knowledge and self-efficacy scores of obese students were lower than those of normal and overweight students;obesity knowledge and attitudes of girls were higher than those of boys,but the self-efficacy scores was lower;students from urban areas have higher obesity knowledge and self-efficacy scores;knowledge,attitude and self-efficacy have increased with the grade.4.Knowledge,attitudes and behaviors of parents of parents: Obesity knowledge and health behaviors of obese parents were lower than those of parents of normal weight and overweight children,parents of girls and from city had higher scores of obesity knowledge,attitude and health behaviors.5.There is a positive correlation between students and parents in knowledge,attitude and health behaviors related to obesity.6.Evaluation of the effect of obesity health education intervention: Before and after intervention,the detection rate of overweight in intervention group decreased by 1.1%and the obesity decreased by 2.5%.Body mass index,waist circumference,body fatpercentage and basal metabolic rate were significantly changed in the intervention group;pupil’s knowledge,attitude,behavior and self-efficacy of obesity in intervention group were improved,the same to the parents.Conclusion:1.The prevalence of overweight and obesity among pupils in Dalian is not optimistic.Boys and students from urban areas should be given priority attention.2.The consistency between the assessment of body size and actual BMI assessment of pupils and their parents needs to be strengthened,especially pupils.3.Pupils and their parents have deficiencies in knowledge,attitude and health behaviors related to obesity,especially pupils,need to be supervised jointly by parents and schools.4.Parents have influence on children’s knowledge,attitude and health behaviors related to obesity.5.Obese health education intervention increases the obesity-related knowledge,attitude and behavior of students and parents,and plays a positive role in prevention and control of childhood obesity. |
