Objective:To investigate the differences of pulse pressure of central aortic pressure and peripheral aortic pressure in order to study the relationship between the differences of pulse pressure and coronary atherosclerosis.Methods: A total of 195 patients who might have coronary heart disease(CHD)from Qingdao Municipal Hospital were chosen,these cases were selected from January 2017 to August 2017,age38-81(62.51 ± 9.15),which include 119 CHD patients and 76 non-CHD patients.I recorded their basic information and collected fasting venous blood to detect biochemical markers.I recorded aortic blood pressure and peripheral blood pressure,which include SBP,DBP,PP,PP=SBP-DBP,PPI=PP/SBP,and calculated the differences of pulse pressure between central and peripheral aortic pressure(medium and external pulse pressure).The patient who has vascular stenosis of coronary artery and main branches showed ?50% by coronary angiography was diagnosed as CHD.These patients are divided into groups of normal patients and CHD patients.According to the coronary artery lesions,they were divided into single and multiple vessel disease groups.Gensini score is used to assess the severity of coronary artery lesion.According to Gensini scores,these patients were divided into three groups.We use SPSS 23.0 software for statistical analysis,including measurement data and counting data.Result:1.The arterial pressure of all patients was satisfied that the systolic blood pressure of central artery was lower than that of peripheral artery,the central artery diastolic blood pressure was higher than that of peripheral artery,and the central artery pulse pressure was lower than that of peripheral blood.The difference was statistically significant(P < 0.05).2.Compared with the normal patients,the proportion of male,smokers and diabetes in CHD group was higher(P < 0.05).There was no difference between two groups in age,hypertension,body mass index,operative heart rate,uric acid and high-density lipoprotein levels between coronary heart disease group and normal group(P > 0.05).For SBP of radial artery,PP of radial artery,PPI of radial artery and difference of medium and external pulse pressure,the group of coronary heart disease was significantly higher than that of the normal group and the difference was significant(P < 0.05).3.Comparison Gensini scores of patients with coronary heart disease,the aortic SBP and aortic PP of group C is higher than group B and group A(P < 0.05);the radial artery PP of group C was significantly higher than group B(P < 0.05).4.Male,smokers,diabetes,total cholesterol level,low density lipoprotein levels of multi vessel group is higher than the normal group.In single vessel group,the proportion of male is higher than the normal group.The number of diabetes mellitus patients in multi-vessel disease group was significantly higher than in single vessel disease group,and the difference was statistically significant(P < 0.05).However,they are same statistically in hypertension,age,BMI,operative heart rate,high-density lipoprotein,serum creatinine and uric acid level among different groups of patients(P > 0.05).The radial artery PP,radial artery PPI and mid-external pulse pressure in the multi vessel disease group were significantly higher than those in the normal group,and the radial artery PPI and mid external pulse pressure in the multiple vessel lesion group were higher than those in the single vessel disease group(P < 0.05).5.Regarded multivessel coronary artery disease as the dependent variable,view the risk factors of sex?age?smoking?diabetes?total cholesterol?LDL?medium and external pulse pressure as independent variables,Logistic regression analysis showed that smoking and diabetes are independent risk factors of multivessel coronary artery disease,which smoking is the most significant one(OR=2.976,95%CI 1.204?7.355,P<0.05).Conclusion:1.The systolic blood pressure and pulse pressure of the central artery were lower than that of the peripheral artery,and the diastolic blood pressure of the central artery was higher than that of the peripheral artery.2.Medium and external pulse pressure in patients with coronary heart disease was significantly higher than that of the normal ones.3.Medium and external pulse pressure of patients with multiple vessel lesions is significantly higher than that of patients with single vessel disease and normal coronary artery.The level of medium and external pulse pressure can reflect the severity of coronary artery disease to a certain extent.4.Smoking and diabetes are independent risk factors of multivessel coronary artery disease,which smoking is the most prominent predictor.
