Objective:Endometriosis(EMs for short)is one of the most common disease of childbearing age women,but the specific mechanism is not clear.This study discussed the change of lipid metabolism and its correlation with the clinical features in endometriosis,through their high density lipoprotein in the blood(HDL-C),low density lipoprotein(LDL-C),triglycerides(TG),total cholesterol(TC),to discuss the effect of lipid metabolism in the process of development of EMs.And thus,new directions are provided for clarifying the possible etiology,treatment and prognosis of EMs.Methods: 213 cases collected in October 2012-October 2017 in dalian obstetrics and gynecology hospital with open or laparoscopic surgery,and after pathologic diagnosis of endometriosis are the EMs group,171 cases without endometriosis are the control group.The age,height,weight,body mass index(BMI),exist dysmenorrhea or not,the stage of endometriosis according to the revision of the American fertility association(r-AFS),exist deeply infiltrating endometriosis(DIE)or not,and HDL-C,LDL-C,TG and TC of those patients were recorded.To compare the change and relationship of HDL-C,LDL-C,TG,TC and BMI in the patients with or without endometriosis.And compared these lipid indexes in the endometriosis divided by r-AFS,DIE and dysmenorrhea.The experimental data were analyzed by SPSS22.0software.Results: 1.The level of HDL-C in EMs group(1.42±0.28mmol/l)was higher than that in control group(1.36±0.28mmol/l),the difference was statistically significant(P < 0.05).The level of TG in EMs group(0.86mmol/l)is lower than that in control group(1.09mmol/l),and the difference was statistically significant(P < 0.05).The level of LDL-C and TC without significant different(P>0.05).2.The BMI in EMs group(21.14±2.68kg/m2)was significantly lower than that in control group(22.06±2.91kg/m2),and the difference was statistically significant(P<0.05).In the endometriosis,the BMI had closed relationship with blood lipid metabolism.There was a negative correlation between BMI and HDL-C level ofEMs patients(r=-0.175,P=0.010),and positive correlation between BMI with TG(r=0.356,P=0.001)and LDL level(r=0.168,P=0.014).3.After grouping by r-AFS(revised classification of American Fertility Society),divided into stage I-II group and III-IV group.The level of LDL-C(2.90±0.58mmol/l)?TG(0.92(0.68-1.06)mmol/l)?TC(4.72±0.73mmol/l)and BMI(21.57±2.90kg/m2)in stage I-II group were obviously higher than that in stage III-IV group(respectively,2.65±0.58mmol/l,0.81(0.62-1.03)mmol/l,4.39±0.74mmol/l,20.81±2.46kg/m2),and the difference was statistically significant(P < 0.05).But there was no statistically significant difference of the level of HDL-C between the stage of I-II group and III-IV group in endometriosis.4.There was no significant difference of the level of HDL-C,LDL-C,TG and TC in the endometriosis patients between the groups by exist DIE or not.5.There was no significant difference of the level of HDL-C,LDL-C,TG and TC in the endometriosis patients between the groups by exist dysmenorrhea or not.Conclusion: 1.There exist abnormal lipid metabolism in the patients with endometriosis.With the progress of the disease,the more serious of the disease,the more aggravating in the lipid metabolism.2.BMI may play important roles in the occurrence and development of endometriosis,and it have close relationship with lipid metabolism.Therefore the lipid metabolism need be focused in the treatment of endometriosis.3.There is no significant correlation in the lipid metabolism between the group whether the endometriosis exist DIE or dysmenorrhea.
