A Comparison Of The Effect Between Zero-Profile Implant System And Anterior Cervical Titanium Plate Combined With Cage System For Single-segment Cervical Spondylotic Radiculopathy

Objective: To evaluate the effect of Zero-Profile implant system compared with anterior cervical titanium plate combined with Cage system for single-segmentcervical spondylotic radiculopathy.Methods:32 patients were retrospectively analyzed,who underwent single-segment anterior cervical discectomy and fusion from February 2013 to January 2016 in gaoyou people's hospital.There are 20 patients applying Zero-Profile implant system(Group A),while 12 patients applying anterior cervical plate system(Group B).Operation time,intraoperative blood loss,JOAscore,VAS,fusion rate of operative level,complications like postoperative dysphagia were compared between two groups.Results: All the operations were completed successfully,and the incisions healed without any serious complications such as infection,major blood vessel and nerve injury,spinal cord and nerve root injury,esophageal and tracheal injuries,and cerebrospinal fluid leakage.The symptoms after surgery were significantly improved compared with before surgery.The operation time in group A was 90.3±11.5min;the operation time in group B was 108.8±13.3min,and there was a significant difference between the two groups(P<0.05).The amount of bleeding in group A was: 52.1±12.3 ml;the amount of bleeding in group B was 63.2±11.4 ml,with no statistically significant difference(P>0.05).Two groups of VAS scores and JOA scores(postoperative 3rd,last follow-up(average 11 months))were significantly higher than before surgery(P<0.05),but there was no significant difference between VAS and JOA in the same period between the two groups(P> 0.05);A group of fusion segment Cobb angle(postoperative 3,the last follow-up(average 11 months))significantly increased than before surgery,the difference was statistically significant(P <0.05),while in group B postoperative fusion segment Cobb angle than preoperative no significant change was found,and the difference was not statistically significant(P>0.05),But the difference between the two groups of A and B was statistically significant(P>0.05);At the last follow-up(average 11 months),both groups met the fusion criteria.The difference in fusion rates between the two groups was statistically insignificant(P>0.05);2 patients in group A(10.00%))there was mild dysphagia,4 patients(33.3%)in group B had dysphagia,and the incidence of postoperative dysphagia in group A and B was statistically significant(P<0.05).Conclusion: The Zero-P internal fixation system combined with anterior cervical titanium plate combined with Cage system has a definite clinical effect on the treatment of single-segment cervical spondylosis.Both of them have a good effect on nerve function recovery and pain improvement,while there was a significant difference between the two groups,the former has advantages of small surgical trauma,short operation time,less intraoperative blood loss,simple operation steps,obvious improvement of cervical lordosis,and low incidence of postoperative dysphagia.It has certain clinical application prospects,but a large sample,long-term follow-up data analysis and research is need.
