Part1:The diagnostic value of different pathological types of gastric cancer by Dual-source CT imagingObjective:To investigate the diagnostic value of dual energy CT imaging of different pathological types of gastric cancer.Methods:50 cases of pathologically confirmed gastric cancer patients in our hospital were divided into gastric cancer group(16 cases)and non mucinous carcinoma group(34 cases)according to the pathological results.Meanwhile gastric mucinous carcinoma were divided into mucinous adenocarcinoma group(5 cases)and signet ring cell carcinoma group(11 cases)and non mucinous carcinoma of stomach were divided into low differentiated adenocarcinoma group(20 cases)and high differentiation adenocarcinoma group(14 cases).Preoperative dual energy spectral CT imagines were obtained including three phase scanning.The original data is transmitted to the workstation.Different pathological types of tissue spectrum curves for gastric cancer were analyzed including its slope through Dual energy analysis software.The slope of the study group was statistically analyzed and compared with postoperative pathology.Results:1.LAttenuation degree of different pathological types of gastric cancer in different keV is different.The curves are all descending types,and the slopes are negative.2.Non mucinous carcinoma compared with mucinous carcinoma group:the curve slope of non mucinous carcinoma was greater,the absolute value of the former was higher than the latter in the three period enhancement curve slope,the difference was statistically significant especially in venous phase and delay phase(P<0.05).3.Signet ring cell carcinoma compared with mucinous adenocarcinoma:signet ring cell carcinoma group has a greater spectrum curve slope,as well the absolute value of the former is higher than the latter to the absolute value of the slope spectrum curve,and the difference between the two energy spectrum curve slope in venous phase and the delay phase were statistically significant(all P<0.05).4.Compared with poorly differentiated adenocarcinoma and well differentiated adenocarcinoma:the former has a more higher spectrum curve slope and a higher absolute value of the slope spectrum curve.The difference between the two energy spectrum curve slope in venous phase was statistically significant(all P<0.05).Conclusion:Different pathological types of gastric cancer has significant difference in the spectral slope of the curve from dual energy spectral CT imaging,dual energy spectral CT imaging can reflect the differentiation degree of pathological tissue and cancer cells of gastric cancer,has an important guiding significance for treatment and gastric cancer evaluation.Part2:Dual energy CT imaging study on gastric cancer T,N stageObjective:Explore the clinical value of dual energy CT imaging on gastric cancer T,N staging.Methods:50 cases of pathologically confirmed gastric cancer patients from our hospital performed preoperative dual energy spectral CT imaging in three phase scanning.The original data is transmitted to the workstation,different T staging of gastric cancer lesions and surrounding lymph nodes normalized iodine concentration(NIC)was enhanced in different phase by dual energy analysis software.Comparing the difference between lymph nodes metastasis and non-metastatic standard iodine value in the different T staging of gastric cancer.Results:1.The NIC value of each gastric carcinoma was statistically significant in late stage of artery phase,portal vein and delayed phase(P<0.05).2.The NIC value of serous outer surface fat clearance in gastric cancer was different between T3 and T4 stage in arterial phase and delayed phase(P<0.05).3.The NIC value in metastatic lymph node and non metastatic lymph node was different in arterial phase and venous phase(P<0.05).Conclusion:There was a difference in the NIC value of serous extracellular fat gap for T3 and T4 gastric cancer in arterial phase and delayed phase.It helps to quantify gastric cancer T3,T4,and have a certain role in distincting between metastatic lymph nodes and non-metastatic lymph nodes.
