Objective:To analyze the correlation between bone mineral density and cystatin C in type 2 diabetes mellitus patients in Yanbian and their common risk factors,and to provide a theoretical basis for the prevention and treatment of osteoporosis and diabetes related complications in T2DM patients.Methods:A total of 344 patients with type 2 diabetes who were hospitalized in Endocrinology Department of Yanbian Hospital from 2016 to 2017 were selected randomly and their clinical data were retrospectively analyzed.The patients were divided into three groups according to the BMD of the selected subjects:normal bone mass group(n=138),osteopenia group(n=116)and osteoporosis group(n=90).Bone mineral density of the right ankle was measured by ultrasonic bone density detector.The gender,age,body mass index,carotid intima-media thickness and blood pressure were recorded.Blood samples were taken for glycosylated hemoglobin,triglyceride,total cholesterol,Lipoprotein cholesterol,low density lipoprotein cholesterol,serum calcium,serum phosphorus,serum cystatin C,blood urea nitrogen,serum creatinine and other biochemical indicators.Measured data using M(P 25,P 75),mean±standard deviation(x±s),non-parametric rank sum test and one-way analysis of variance for comparison between groups,linear regression for trend test,multi-factor analysis Using multiple logistic regression analysis,P<0.05 was statistically significant.Results:1.In 344 T2DM patients in Yanbian,the prevalence of osteoporosis was 26.2%,the prevalence of osteopenia was 33.7%,and the prevalence of normal bone mass was 40.1%.BMD levels decreased with age(P<0.05).BMD levels in normal subjects were lower than those in overweight and obesity groups,and BMD levels increased with the increase in BMI levels(P<0.01).The systolic blood pressure in patients with osteoporosis was higher than that in the bone reduced group and the normal bone group(P<0.05).The diastolic blood pressure in patients with osteoporosis was lower than that in the bone reduced group and the normal bone group(P<0.05).T2DM patients have lower BMD levels than women.Carotid intima thickening group and carotid atherosclerotic plaque group had lower BMD level than normal carotid artery group.The TC of patients with osteoporosis was lower than that of the bone reduced group and the normal bone group,and the difference was statistically significant(P<0.05).CysC decreased with the increase of BMD level(P<0.01).2.There was a positive correlation between BMI,DBP and BMD.The negative correlation between CysC,TC,age and bone density was an independent risk factor of OP.The difference was statistically significant(P<0.05).3.P,BUN,Cr,SBP and CysC levels were positively correlated(P<0.01),while LDL,BMD and CysC levels were negatively correlated(P<0.01),the remaining indicators had no correlation(P>0.05).Conclusion:1.BMD was negatively correlated with CysC level in T2DM patients,and CysC was an independent risk factor for osteoporosis.2.BMD in T2DM patients is closely related to age,gender,atherosclerosis,BMI,TC,and blood pressure.BMI,DBP were positively correlated with bone mineral density.Age and TC were an independent risk factor for OP.3.CysC in patients with T2DM and phosphorus,renal function,blood pressure,LDL are closely related.
