With the increasing number of type 2 diabetes mellitus,various complications have attracted much attention.Bone is becoming the target organ of complications of type 2 diabetes mellitus.Type 2 diabetes mellitus is a predisposing group of osteoporosis(OP),with an increased risk of fracture.However,the mechanism of osteoporosis in type 2 diabetes mellitus remains unclear.?-arrestin2 is a binding protein with multiple functions,which can regulate a large number of cellular functions and has certain effects on diabetes mellitus and bone metabolism.Objective:To explore the correlation between serum ?-arrestin 2 level and bone mineral density in type 2 diabetes mellitus,and to provide evidence for the prevention of osteoporosis in type 2 diabetes mellitus..Methods:A total of 178 patients with type 2 diabetes who were hospitalized in our hospital from January 2018 to December 2018 were collected.The general data of each patient's name,sex,age,systolic blood pressure,diastolic blood pressure,height,weight,BMI and course of diabetes mellitus were collected.At the same time,the liver and kidney function,blood lipid,blood calcium,phosphorus alkaline phosphatase,PTH and other biochemical indicators were detected.Bone turnover markers: total N-terminal prolongation of type I collagen(tPINP),specific sequence of ?-collagen(?-CTX),osteocalcin(OC),25-hydroxyl vitamin D(25-OH-VD)and other bone turnover markers(BTMs)were measured.Measured serum levels of ?-arrestin2.Lumbar L1-4 and hip bone mineral density were measured by dual energy X-ray absorptiometry.According to the bone mineral density,the patients were divided into normal bone mass group,low bone mass group and osteoporosis group.The levels of serum ?-arrestin2 were compared in each group,and the correlation between ?-arrestin2 and bone turnover markers tPINP,?-CTX,OC,25-OH-VD and ALP was analyzed.Logistic regression was performed with age,BMI,glycated hemoglobin,triglyceride,total cholesterol,serum calcium,ALP,tPINP,?-CTX,OC,25-OH-VD and PTH as independent variables to determine the independent risk factors of osteoporosis in type 2 diabetes mellitus and the correlation between serum ?-arrestin2 and bone mineral density.Results:1.General clinical data: There was no significant difference in gender,BMI,glycated hemoglobin,C-peptide,C-peptide 2h,blood potassium,blood sodium,blood calcium,triglyceride,total cholesterol,creatinine,course of diabetes and PTH among the three groups(P>0.05).The age of osteoporosis group was significantly higher than that of normal bone mass group(P> 0.05).2.Comparison of serum ?-arrestin2 among three groups: serum ?-arrestin2 was 1.46 ng/mL in normal bone mass group,1.05 ng/mL in low bone mass group and 0.80 ng/mL in osteoporosis group.The level of serum ?-arrestin 2 in low bone mass group and osteoporosis group was significantly lower than that in normal bone mass group(P<0.05),and there was no significant difference between low bone mass group and osteoporosis group(P>0.05).3.The correlation between serum ?-arrestin2 and bone mineral density ?bone turnover index: serum ?-arrestin2 was positively correlated with bone mineral density(?=0.26,P=0.03).It was negatively correlated with ALP(?=-0.20,P=0.04)and positively correlated with 25-hydroxyl vitamin D(25-OH-VD)(?=0.22,P=0.01).There was no significant correlation between serum ?-arrestin2 and total tPINP??-CTX? OC in bone turnover index(P>0.05).4.Osteoporosis in type 2 diabetes mellitus: Age is an independent risk factor for type 2 diabetes mellitus with osteoporosis.Serum ?-arrestin2 is a protective factor for osteoporosis in type 2 diabetes mellitus(OR=0.44,95% CI(0.27-071),P < 0.05).Conclusion:Serum ?-arrestin2 is positively correlated with bone mineral density,negatively correlated with ALP and positively correlated with 25-OH-VD in type 2 diabetes mellitus,suggesting that serum ?-arrestin2 can affect bone mineral density in type 2 diabetes mellitus and is a protective factor for osteoporosis in type 2 diabetes mellitus.
