| Objective:To compare the clinical and laboratory data of intrahepatic cholangiocarcinoma(ICC)and hepatocellular carcinoma(HCC),so as to enhance the awareness of ICC and provide a theoretical basis for differentiating ICC from HCC.Methods:Using the retrospective analysis method,collecting from January 2011 to December 2017 in the Affiliated Hospital of Yanbian University diagnosed with ICC 78 patients,and by using the random number table.Randomly selected 511 patients diagnosed with HCC,statistical analysis of the patients’ gender and age,nationality,main symptoms,drinking history,chronic viral hepatitis history,history of biliary’s disease,tract tumor markers.The measurement data were expressed as(x±s),and t test was used for comparison between groups.The percentage of counting data was expressed as a percentage,and the comparison of the composition ratio between groups was compared with the χ2test.P<0.05 was statistically significant.Results:HCC and ICC,the incidence rate increased with age,before the age of 40,HCC and ICC incidence is similar,after 40 years of age,the incidence was statistically different between different age group.61-71 year old women are ICC high-risk group,51-60 year old male are HCC high-risk group;ICC had a history biliary disease but without hepatocirrhosis,abdominal pain,upper abdominal discomfort and jaundice as the main symptoms,liver function is generally better;there was no significant difference in NLR,PLR and LMR between ICC and HCC patients,peripheral blood CEA and CA-199 levels were significantly higher than those in patients with HCC,AFP does not rise,but there are many HCC had a history of chronic viral hepatitis,most patients have cirrhosis of the liver at the onset of the disease,the main symptoms of fatigue,anorexia,ascites,edema of lower extremities and reduce the amount of urine,liver function decompensation.The levels of CEA and CA-199 in peripheral blood were not high or slightly higher,and the level of AFP was significantly higher than those of ICC,and the difference was statistically significant.Conclusion:1.Both HCC and ICC are malignant tumors of the liver,but their clinical features,mechanism and biological characteristics are very different.They need to be treated differently.2.Most of the patients with ICC had no liver cirrhosis,and the liver function was mostly in the Child-Pugh A level.The pathogenesis was chronic intrahepatic cholangitis lead to ICC3.Liver tumor patients combined with AFP,CEA,CA-199 and liver function tests are helpful to distinguish between HCC and ICC. |
