Objective:Detection of serum AFP,CA19-9,CA125 and CEA levels were observed in hepatocellular carcinoma,intrahepatic cholangiocarcinoma and hepatic hemangioma differential diagnosis.Methods:The patients in our department in line with the conditions of patients were analyzed retrospectively,including 144 cases of hepatocellular carcinoma, intrahepatic cholangiocarcinoma in 34 cases,42 cases of hepatic hemangioma, serum samples of their electricity chemiluminescence immunoassay method AFP,CA19-9,CA125 and CEA levels.Results: 1.Serum AFP in hepatocellular carcinoma,intrahepatic cholangiocarcinoma in the liver hemangioma test results:serum AFP for hepatocellular carcinoma patients(5807.85±14578.48) ng/ ml,median 93.18 ng/ ml,intrahepatic for cholangiocarcinoma(4.15±2.86) ng / ml,a median of 3.72 ng/ ml;hepatic hemangioma was(2.86±1.45) ng / ml,a median of 2.73 ng / ml.AFP were compared in each group the number of hepatocellular carcinoma was significantly higher than intrahepatic cholangiocarcinoma hemangioma and liver, P<0.05,but the bile duct cancer cells compared with hepatic hemangioma,no significant statistical difference,P>0.05.2.Serum CA19-9 in hepatocellular carcinoma,intrahepatic cholangiocarcinoma hemangioma liver test results in:hepatocellular carcinoma patients with CA19-9 for(48.67±147.42) u / ml,median 14.92u / ml;for intrahepatic cholangiocarcinoma(600.87±1168.31) u / ml,a median of 273.53 u / ml; hepatic hemangioma was(7.10±9.52) u / ml,a median of 4.21 u / ml. Comparison of CA19-9 in each group are the number 22 among the groups of comparison,the P<0.05,indicate statistical significance,but the intrahepatic cholangiocarcinoma was significantly higher than that of hepatocellular carcinoma and liver hemangioma.3.Serum CA125 in hepatocellular carcinoma,intrahepatic cholangiocarcinoma in the liver hemangioma test results:serum CA125 for patients with hepatocellular carcinoma(37.09±134.49) u / ml,a median of 12.03 u / ml; intrahepatic for cholangiocarcinoma(157.51±193.02) u / ml,a median of 65.24 u / ml;hepatic hemangioma was(13.47±18.82) u / ml,a median of 7.55 u / ml. Comparison of each group are a number of CA125,intrahepatic cholangiocarcinoma was significantly higher than that of hepatocellular carcinoma and liver hemangioma,P<0.05 or P<0.01.However,hepatocellular carcinoma and hepatic hemangioma no significant statistical difference,P>0.05.4.Serum CEA in hepatocellular carcinoma,intrahepatic cholangiocarcinoma in the liver hemangioma test results:for hepatocellular carcinoma in patients with (2.36±2.96) ng / ml,a median of 1.74 ng / ml;intrahepatic bile duct cell carcinoma was(23.20±48.21) ng / ml,median 4.85ng/ml;hepatic hemangioma was(1.58±1.05) ng / ml,median 1.34ng/ml.Comparison of each group are a few of the CEA,intrahepatic cholangiocarcinoma and hepatocellular carcinoma is not statistically significance,P>0.05,compared with hepatic hemangioma,P<0.05,hepatocellular carcinoma and hepatic hemangioma comparison,P<0.05. However,intrahepatic cholangiocarcinoma were significantly larger than the other two tumors.5.Hepatocellular carcinoma with elevated AFP mainly,intrahepatic cholangiocarcinoma with CA199,CA125 and CEA increased mainly,hepatic hemangioma four markers were within the normal range. 6.Under the ROC curve to determine,serum AFP has the diagnosis of hepatocellular carcinoma,CA19-9,CA125,CEA for the diagnosis of hepatocellular carcinoma of little value;CA19-9,CA125,CEA cell carcinoma of the intrahepatic bile ducts have diagnostic value,AFP of intrahepatic cholangiocarcinoma no diagnostic value;four markers for diagnosis of hepatic hemangioma of little value.Conclusion:The combined determination of serum AFP,CA19-9,CA125,CEA in the differential diagnosis of hepatocellular carcinoma,intrahepatic cholangiocarcinoma and hepatic hemangioma has an important significance.
