Periodontitis is a microbial infectious disease.Research shows that periodontitis is closely related to systemic disease.In recent years,periodontal disease has close relationship with respiratory diseases,especially the relationship with chronic obstructive pulmonary disease(COPD)has obtained more and more attention.COPD,a kind of respiratory system disease,which is characterized by an incomplete reversible air flow.Studies have shown that periodontal pathogens such as gingival porphyrins,nucleobacterium,actinobacterium,and intermediate plutella can stimulate the production of cytokines in respiratory epithelial cells,while inflammatory factors such as interleukin 6(IL-6),interleukin 8(IL-8)are believed to play an important pathological role in the development of the disease.The cytokine NF-?B,produced by the respiratory system,activates the IL-8 gene in the epithelial cells and macrophages to activate the positive feedback effect.IL-6 is a multipotent cytokine.On the one hand,IL-6 participates the inflammatory response and immune response of periodontitis,induces osteoclast formation,continuous aggregation and activite osteoclast and aggravate the destruction of periodontal tissue again.On the other hand,IL-6 can activate neutrophils and promote the release of TNF-? and interferon by monocytes and T cells which causes the injury of bronchoalveolar mucosa,promotes the formation of immune adhesion and microthrombus,aggravates the degree of inflammation of COPD and the difficulty of breathing.IL-8,the most important inflammatory medium in the pathogenesis of COPD,is involved in the destructing and absorbing of the alveolar bone,in the process of the progress of periodontal disease,its overproduction led to excessive activation of neutrophils,not only directly destruct periodontal tissue,further affect the host's inflammatory response and immune process.This paper chose the patients with chronic periodontitis and COPD for research to collect general information,to detect the levels of IL-6 and IL-8 in serum,to explore the role of IL-6 and IL-8 in the pathogenesis of chronic periodontitis and COPD,to investigate the possible correlation between chronic periodontitis and COPD,and to provide more effective clinical new ideas for the combination therapy and collaborative treatment of the two diseases.Objective: To investigate the serum levels of IL-6 and IL-8 in patients with chronic periodontal disease and acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and the possible relationship between the two diseases.Methods: A total 40 cases of healthy subjects(control group 1)and 120 cases of eligible patients were collected,40 were patients with chronic periodontal disease and AECOPD(experimental group),40 were patients with chronic periodontal disease(control group 2),40 were patients with AECOPD(control group 3).ELISA assay was used to detect the serum levels of IL-6 and IL-8 in the four groups.Results:1.General information of the experimental group and control groups.General information of selected subjects were collected,including age,gender and body mass index(BMI).The statistical results showed no significant difference in BMI and gender in each group.There were no significant difference between the experimental group and control group 2 and control group 3 in age.The age of control group 1 were significant different from other groups.2.Periodontal index of the experimental group and control groups.Periodontal index of selected subjects were detected,including tooth mobility degree,probing depth(PD),sulcus bleeding index(SBI)and attachment level(AL).Statistical results show that the tooth mobility degrees of experimental group and control group 2 were ? to ?,had no significant differences.There were no significant difference between the experimental group and control group 2 in PD,SBI and AL.The periodontal index of control group 1 were significant different from experimental group and control group 2.3.Lung function index of the experimental group and control groups.Lung function index of selected subjects were detected,including vital capacity max(VC Max),forced expiratory volume in one second(FEV1),forced expiratory volume in one second/forced vital capacity(FEV1/FVC).The VC Max,FEV1 and FEV1/FVC of experimental group had significant different from control group 3.The VC Max,FEV1 and FEV1/FVC of control group 1 were significant different from experimental group and control group 3.4.Expression levels of serum IL-6 and IL-8 in the experimental group and control groups.Detected the serum levels of IL-6 and IL-8 in the experimental group and control groups.The results showed that the serum IL-6 and IL-8 levels in the experimental group were significantly higher than that in the control group 1,2 and 3(P<0.0001).The levels of IL-6 and IL-8 in the control group 2 and 3 were significantly higher than that in the control group 1(P<0.0001).Conclusions:1.Chronic periodontitis and AECOPD may interacte and influence each other.2.The IL-6 and IL-8 levels of experimental group were significantly higher than those in control group.IL-6 and IL-8 may be associated with the pathogenesis of periodontal disease and AECOPD closely.
