Preliminary Study On The New Training Model Of Primary General Practitioners

Objective With the development of new medical reform and graded diagnosis and treatment,new demands are putting forward on the basic level general practitioners'health service ability.The general practitioners with the basic level health service ability are the basis of realizing the new medical reform.Nowadays,medical knowledge is constantly updated rapidly.In order to improve the ability of health service and keep the door of people's health,primary general practitioners must constantly continue studying new knowledge to improve their basic medical service competence.However,at present there is a shortage of medical personnel in primary health units and heavy work tasks,and it is difficult to achieve long-term out-of-work study.It is even very difficult to attend continuing education classes for 1-2 days during the working period.The contradiction between work and study is very prominent,which seriously affects the knowledge renewal and competence of medical personnel.To speed up the training of qualified general practitioners and improve the health service ability of general practitioners at the primary level is one of the important tasks of our general practice.How to overcome the contradiction between effective work and learning is the key and difficult point in current general practice research.As a teaching hospital and a standardized training base for general residents in Anhui medical colleges,our hospital has the responsibility and obligation to make contributions to the cultivation of high quality and high quality general practitioners in Anhui Province.By innovating the new mode and practice of continuing education after graduation of primary general practitioners on the job,this paper follows up and summarizes the advantages and disadvantages of the new mode of training primary general practitioners.It provides the basis for the study of overcoming the contradiction of engineering and learning in the continuing education of primary general practitioners after graduation.Methods In view of the contradiction between engineering and learning in the updating of knowledge among doctors at the primary level,we should innovate the "unlimited time,way,content and charge" new mode of "si bu yi bei" for primary doctors to study in the third Class A Hospitals of the General discipline training Base.According to the practical problems and the need of renewing knowledge in the work of the basic level doctors,the individualized learning scheme of "what is lacking and what to fill" is formulated,and the teachers with general course teaching experience are selected to carry out the "one to one" teaching.To collect the basic information of the basic doctors who have been registered in our hospital for 5 years and complete their further study,and to investigate and return the students by questionnaire and telephone follow-up,and to collect the experience,feelings and opinions of the trainees after further study.A questionnaire survey was conducted at random to collect the experience,perception and opinions of the learning method.Results In addition to the students who had not completed the program,17 valid questionnaires were collected in the past five years,and 9 teachers were randomly surveyed.11.8%of students majoring in ECG,29.4%of students majoring in B-ultrasound,11.8%of students majoring in cardiovascular medicine,5.9%of students majoring in endocrinology,17.6%of undergraduates majoring in obstetrics and gynaecology,17.6%of those majoring in pediatrics,and accounting for continuing majors in oral medicine 5.9%.Among the advanced learning students,5.88%were engaged in electrocardiogram major;29.4%were three months of advanced education;17.6%were six months;23.5%were nine months of study;29.4%were a year.11.76%of the original majors are general subjects;17.65%are engaged in pediatrics;5.88%are engaged in physical examinations;5.88%are engaged in oral cavity;23.53%are engaged in B-ultrasound;11.76 are engaged in internal medicine.%;engaged in obstetrics and gynecology accounted for 17.65%.In the learning process,41.2%of the shifts were scheduled and 58.8%were assigned to any schedule.66.7%of the leading teachers have participated in general practice training,and obtained the qualification of general practice teachers,and 100%of the teachers have certain experience in teaching basic level doctors,and they are willing to do their best to improve the clinical competence of primary doctors.To actively participate in "one-to-one" clinical teaching,hope to be able to communicate with doctors who study at the primary level,understand the current situation of primary health units and the needs of primary doctors,and better participate in the work of general discipline training.Fully affirm the new model of "si bu yi bu" and hope to train qualified general practitioners to make their own contribution.Conclusion Our country is now in the key period of the reform of medical and health system.With the development of new medical reform and graded diagnosis and treatment,new requirements are put forward for primary health service.Qualified general practitioners are the basis of the new medical reform.At present,the main body of general practitioners is still working at the grass-roots level of general practitioners.So,it is urgent to solve the contradiction between the renewal of the knowledge level of the primary doctors and the heavy work at the rural level.Our innovative new mode of continuing education for primary doctors after graduation is in line with the actual situation of our country's current primary health units,effectively solves the contradiction of "working and learning",and aims at the needs of primary doctors,so as to be flexible and flexible.Our innovative "si bu yi bei" basic education model for post-graduate doctors graduating to meet the actual situation of China's current primary health units,mainly addressing the needs of primary practitioners at the grassroots level to study,effectively solve the "work,contradiction,for the needs of primary doctors,be flexible,lack what to make up,and effectively improve the clinical competence of doctors at the grass-roots level,and get the basic acclaim of the doctors at the grass-roots level;while carrying out the "si bu yi bei" model,but also improve the general discipline training base to teach teachers on the grass-roots level The current status of health units and the needs of grass-roots doctors,and the promotion of the general teaching level of teaching hospitals are worth promoting.
