| Objective1.Through a cross-sectional survey to understand the current status about caregivers'benefit finding of chronic heart failure patients.2.To analysis the influencing factors about caregivers'benefit finding of chronic heart failure patients.MethodsFrom October 2016 to February 2017 totally 280 couples of Chronic Heart Failure patients and caregivers were meet our standards and we investigated from The First Affiliated Hospital of Zhengzhou University by convenience sampling method.To collect data we use the General Information questionnaire:Activity of Daily Living Scale(ADL),benefit finding scale(BFS),caregiver burden inventory(CBI),social support rating scale(SSRS),Satisfaction with life Scale(SWLS),Simplified Coping Style Questionnaire(SCSQ).The Epidata3.0 software was used to record the data of the recycled questionnaire,and then the SPSS21.0 statistical analysis software was used to analyze the data.The general data were statistically described.Independent samples t test,one-way ANOVA,Pearson correlation analysis and multiple linear regression analysis were used to explore the related factors that influence the discovery of caregivers'benefits.P<0.05 was considered statistically significant.Results1.Questionnaire collection:A total of 300 questionnaires were distributed to the person who met the inclusion criteria in this survey.280 valid questionnaires were returned and the effective recovery rate was 93.3%.2.General information for patients and caregivers:Male and female patients were comparable;Patients aged between 24~94 years old,with an average age of(62.4±12.2)years of age;Married individuals accounted for 85.4%;Most of the patients had a junior high school or below education level,accounting for 74.3%;221patients(78.9%)without religion;139(49.6%)patients were farmers.58.2%of the patients had comorbidities,which accounted for the majority;Classification of cardiac function in grade II in 130 cases(46.6%)and 107 cases of grade III(38.2%);The patients'shortest course was one month,and the longest was up to 30 years.There were 130 cases(46.4%)with illness less than 3 months and 124 cases(44.3%)with>12 months of illness.Medical expenses payment method:146 cases(52.1%)of the new rural cooperative medical system accounted for the largest proportion.Among the caregivers,154(55.0%)were male caregivers;Caregivers aged between19 and 77 years old,the average age was(43.5±13.3)years,and the majority population age were under 60 years old,accounting for 93.6%;Accounted for 91.1%caregivers were married;The educational level was 59.3%mainly for senior middle school or above;237 cases(84.6%)caregivers had no religious belief;Caregivers accounted for 43.6%of the working population;Family income was less than 4,000yuan,accounting for 83.5%;245 cases(87.5%)caregivers did not have complications;Children were the main caregivers,accounting for 61.8%;The care of patients with time less than 3 months at most,204 cases(72.9%).3.The total score of BFS of patients with Heart Failure was(76.0±17.3),The highest score was the average of the health behavior dimension(3.7±1.2),the family dimension(3.7±1.1)and the personal growth dimension(3.5±0.9),The lower dimension is the world's latitude(3.0±0.8)points,accepted latitude(3.4±1.1)points,Social relations dimension(3.4±1.0)points.4.After statistical analysis variables related to caregivers'benefit finding:it was found that caregivers'gender,age,presence of other diseases,relationship between caregivers and patients,the length of care patient,patients'medical insurance category were related to caregivers'benefit finding,the statistical significance(P<0.05);The total score of social support(r=0.401)and social objective support positively correlated with benefit finding.The r values of the three dimensions of subjective support and social utility were 0.437,0.367,and 0.484,respectively;the total score of benefit finding for caregivers was inversely related to the developmental constraints in caregiver burden(r=-0.154),and sociality.The burden was positively correlated(r=-0.127);positively correlated with life satisfaction(r=0.157),positively correlated with the simple coping style questionnaire(r=0.361),and positively correlated with the positive coping dimension(r=0.428).5.Factors influencing the caregiver's benefit finding:There are eight variables enter the regression equation that affect the caregivers'benefit finding.It can be seen that the influencing effect of benefit finding level is the positive response dimension of the simple coping style,the socially restricted dimension and developmental restriction dimension of Caregiver Burden Inventory,caregivers'age,relationship between caregivers and patients,R ~2=0.46,and the validated regression equation model was statistically significant(F=9.387,P<0.001).Conclusion1.The level of benefit finding for caregivers of patients with chronic heart failure is on average and needs to be further improved.The top three in the six dimensions are:health behavior dimension,family dimension,and personal growth dimension.2.The factors affecting the benefit finding of caregivers mainly include:the positive coping dimension in the simple coping style,the age of the caregivers,the development constrained dimension in the caregiver's burden,the socially restricted dimension,and the relationship between the caregiver and the patient.3.Medical staffs can guide the caregivers from a positive point of view and improve the level of their benefits based on the results of the benefit finding analysis. |
