The Association Between Filial Piety Values And The Burden Of Caregivers For Taking Care Of Chronic Heart Failure Patients

Part 1 Reliability and validity analysis of Chinese Filial Values Index scaleObjective: To introduce the Filial Values Index(FVI)scale and form Chinese FVI scale through the scale localization program including translation,back translation,correction and expert consultation,then evaluate reliability and validity of the scale.Methods: On the basis of obtaining the approval of the original author,we translated the English FVI into the Chinese FVI,and translated the Chinese FVI into the English FVI according to the scale of localization process.Ten caregivers of chronic heart failure(CHF)patient were selected to make a preliminary investigation,and the original Chinese FVI scale was formed by overall expert consultation again.Adopting convenient sampling method,we selected 98 caregivers taking care of CHF patients in 3 hospitals from March to October 2016 as the research object,and investigated their demographic data and filial values.During the study,20 caregivers had a resurvey of filial values within one week.The SPSS18.0 software was adopted to establish the database and statistical analysis of data.We used critical ratio and correlation coefficient for the analysis of project,and the principal component and exploratory factor analysis was used for the factor analysis and selection.At the same time,we used the Cronbach's a coefficient to assess the internal consistency,and the expert consultation and principal component factor analysis were the evaluation indexes of the questionnaire content validity and structure validity.Results:(1)After the initial scale of translation and back translation and expert content validity evaluation,the Chinese FVI scale was formed.The scale score was 0.67 ~ 1.00,and the average score was 0.89;(2)The critical ratio and correlation coefficient showed that the scale item discrimination ability is outstanding within two groups of high or low scores(P < 0.05).The correlation coefficient between each item and total score mostly was 0.4 ~ 0.8 in addition to item 1 and item 9 or 12 whose correlation coefficient is 0.36,and,indicated that the entry had good homogeneity and representative;(3)Chinese FVI scale was extracted two common factors(eigenvalue> 1)by exploratory factor analysis which could explained 68.91% of the total variance,and the load coefficient of each item on the two factors on was greater than 0.45,which mean that the factor structure is reasonable;(4)The content validity of the scale and factors was 0.89~0.94,and the retest reliability was 0.97,which indicated the Chinese FVI scale has good reliability and validity.Conclusion: Chinese Filial Values Index(FVI)scale had good reliability and validity,which can be used for evaluation of domestic caregivers filial piety values for CHF patients.Part 2 The association between filial piety values and the burden of caregivers for taking care of chronic heart failure patientsObjective: To investigate the status quo of burden for taking care of the CHF patient of caregivers,and analyze the influence factors that contribute to it,especially the filial piety values.Methods: This study was a descriptive research,and adopted the convenient sampling method.192 caregivers meting the inclusion and exclusion criteria were selected to finish the questionnaire investigation from March to December 2016 in three hospitals.The questionnaire includes patients and caregivers general situation questionnaire,the Chinese FVI scale,Dutch Objective Burden Inventory(DOBI),relationship quality score,visual analogue scale(VSA)anxiety score and depression two-item Patient Health Questionnaire(PHQ).SPSS18.0 software was used to do data entry and variable correlation analysis,and then using AMOS 22.0 to model the structural equation and adjustment.It is credible to analyze the direct and indirect influence factors of the caregiver burden by path analysis.Results: 64.1% of CHF patients caregivers had the object caregiver,and the practical burden got the highest scores.The path analysis of caregiver burden factors showed that:(1)The direct factors of caregivers from the patients included the number of accompanying diseases,readmission rate in half year and degree of anxiety,and the total effect value were 0.31,0.20,0.36 separately(P < 0.05);indirect influence factors included cardiac function level,depression,and the average length of hospital stay;(2)The direct factors of caregivers from the patients included caregivers filial piety values,the degree of anxiety,mean time of taking care of the patients and the perception to the disease and the total effect value were-0.38,0.29,0.29 and-0.23 separately(P < 0.05);the indirect factors included their attitude toward disease and the relationship quality between the patient and caregiver.(3)The filial piety of offspring caregivers obviously had more influence on caregiver burden,whose direct effect value was-0.41(P < 0.001).Conclusion: Most of the caregivers could feel the burden of take care of the CHF patients,which had various and complicated factors.The values of filial piety were the main factor influencing the caregiver burden,which provided reliable theory basis for setting the intervention of caregivers' burden.
