Clinical Study On Treatment Of Ulcerative Colitis With Jianpi Qingre Huoxue Decoction From NLRP-6,Caspase-1 And IL-18

Object Based on the changes of NLRP-6,Caspase-1 and IL-18 expression to explored the possible mechanism of "Jianpi Qingre Huoxue Decoction" in treating ulcerative colitis.Methods According to the randomized controlled study design method,30 patients with mild-moderate inclusion ulcerative colitis were randomly divided into treatment group and control group,with 15 cases in each group.The treatment group was given Jianpi Qingre Huoxue Decoction oral(Prescription composition:jiubiying 15 g,sanqi 8g,baishao 12 g,machixian 12 g,baizhu 12 g,shuizhi 6g,zhigancao 6g),the control group was given Mesalazine granules orally for 12 weeks.The curative effect of the two groups was evaluated by colonoscopy and Baron score,the ulcerative colitis TCM syndrome(abdominal pain,diarrhea,sepsis,and tenesmus)were evaluated,NLRP-6,Caspase-1,IL-18 expression changes.Results(1)There was no significant difference between the two groups in overall effective rate,The total effective rate was 80.0% in the treatment group and 73.3% in the control group,The treatment group completely relieved in 2 cases,markedly effective in 3 cases,effective in 7 cases,ineffective in 3 cases,control group completely relieved in 1 case,markedly effective in 2 cases,effective in 8 cases,ineffective in 4 cases,no statistical difference(P> 0.05).(2)The value of Baron score under colonoscopy before treatment(treatment group5.42±1.46 andcontrol group 5.27±1.51),after treatment,colonoscopy Baron score integral values were(treatment group3.47±1.37 andcontrol group 3.85±1.71),There was no significant difference between the two groups before treatment(P>0.05),after treatment,the integral values of both groups were decreased,There was a significant difference between before and after treatment in two group(P <0.05),but no significant difference between the treatment group and the control group after treatment(P>0.05).(3)The scores of TCM syndromes in both groups decreased to varying degrees after different interventions.The abdominal pain,tenesmus,and pus scores in the two groups were compared at weeks 4,8,and 12 after treatment and before treatment.Statistical differences(P<0.05),diarrhea scores were statistically different at 8 weeks and 12 weeks after treatment compared with before treatment(P<0.05).The intersubjective effect test showed no statistical difference between the groups(P>0.05).(4)The expression of NLRP-6,Caspase-1,IL-18 positive cells and immunohistochemical positive cells distribution area in two groups before treatment were almost the same,with no significant difference(P> 0.05).After treatment,the detection of 12 was found to be reduced to some extent before treatment,which was more obvious in the treatment group.There was significant difference between the treatment group and the control group after treatment(P <0.05).Conclusion Jianpi Qingre Huoxue Decoction can reduce NLRP-6 expression,inhibit Caspase-1 activation,down-regulate the expression of IL-18 and exert its therapeutic effect on ulcerative colitis,which is worthy of clinical application.
