Analysis Of Tongue Image And TCM Syndrome Type And Related Factors In Hypertensive Patients

Objective:To explore the distribution rules and characteristics of the tongue image and TCM syndrome and related factors of hypertension,and to provide a theoretical basis for the syndrome differentiation and treatment of clinical hypertension.Method:Collecting data about 398 patients with essential hypertension by questionnaire survey,such as:general information,medical history,tongue diagnosis,clinical symptoms and signs of traditional Chinese medicine,laboratory examination and so on.The relevant standards refer to the 2010 edition of the guidelines for the prevention and control of hypertension in China and the guiding principles for clinical research on new drugs of traditional Chinese medicine in 2002.The Logistic regression analysis,correspondence analysis and statistical analysis is made through SPSS19.0 and Excel2010.Result:A total of 398 cases are investigated,of which 153 is male(38.3%)and 245(61.6%)is women.The proportion of men and women is 1:1.6,and women are higher than men.The age was between 33~91 years and the average age was 68.05±10.286 years old.The number of people aged 60~89 is the most,with a total of 322 people(80.9%).The course of 1~5 years was 163 cases(41%),115 cases(28.9%)in 5~10 years and 72 cases(18.1%)in 10~15 years.The blood pressure grade 1 was the most,174(43.7%),followed by grade 2,106(26.6%),86(21.6%)in normal(21.6%)and 32(8%).The distribution of body mass index: 169 cases(42.5%)of normal weight,149 cases of overweight(37.4%),and 63 cases of obesity(15.8%).(1)Gender The munber of pink tongue,pale tongue,dark red tongue is largest.The number of women is more than men(P>0.05).Crack tongue,normal tongue,tooth marked tongue is most;which women is more than men(P < 0.01).Under the tongue bending with collaterals purple color thick swelling normal sublingual collaterals,male tongue blood stasis silk,petechiae more than female,more female than male(P <0.05).Thin white fur and white greasy fur is most,thin yellow tongue,yellow greasy tongue fur men is more than women,others women than men(P < 0.01).(2)Age Pale tongue,tongue crack,normal almost is elderly and normal tongue,tooth marked tongue are among the middle-aged(P < 0.05).More about,as the age increases,it increases first and then decreases.Dark red tongue,sublingual vein with color purple,rough expansion,bending in the elderly(P > 0.05),The majority of young and old people had thin white tongue and white greasy fur at different age groups(P > 0.05).(3)Course of disease In 1~5 years,5~10 years,more normal tongue crack tongue,tongue with crack and spot prick more in.10~15 years.more.But,in 15~20 years,spot prick tongue,thin tongue crack tongue,tongue more normal are more than others(P < 0.01).It suggests that with the increase of the course of disease,there was a trend of changing from a single tongue to a phase with tongue,The tongue coating decreases with the increase of the course of the disease..(4)Blood pressure From the survey pink tongue is most,red tongue,dark red tongue is second,normal tongue,cracked tongue,tooth marked tongue is also mostly,and with the increase of blood pressure,the number is decrease.The hypoglossal veins were normal,the color purple was thick and bulging,and the 1 was most(P < 0.01).Tongue is based on thin white fur,white greasy fur.(P > 0.05).The standard is normal of UA,HDL-C,ALT,Cr,TC,in different tongue color,ligulate,sublingual vein,coating on the tongue.The LDL-C,TG,FBG are unusual,in the red tongue,dark red tongue,thin tongue,prick tongue,sublingual vein Color Purple swelling,bending coarse greasy fur.It is suggested that abnormal blood lipid has a great influence on the incidence of hypertension.It is more important to explain the pathological factors of blood stasis and phlegm from the point of view of traditional Chinese medicine.There is no obvious correspondence between the distribution of tongue color,sublingual vein and TCM syndrome type.Kidney deficiency is prone to prick the tongue,and tongue,thin white fur,phlegm and blood stasis is prone to crack tongue,yellow greasy fur.The red tongue,the toothed tongue and the tensioning bundle of the sublingual color are related to the total cholesterol.The Pale tongue,pale red tongue,fissured tongue,sublingual vein Color Purple rough expansion bend,sublingual vein color green are correlation with age.The Pale tongue,fissured tongue are related to smoking.The toothed tongue is associated with high density lipoprotein cholesterol and low density lipoprotein cholesterol.It is related about normal tongue and body weight,white greasy fur and fasting glucose,thin white coating,thin yellow coating and creatinine related.Conclusion: The characteristics of tongue in patients with hypertension show that the disease is characterized by deficiency and excess.The distribution of tongue in different sex,age and duration of hypertension is quite different.The abnormal of LDL-C,TG and FBG have an effect on the tongue image.Tongue of hypertension patients is related to syndrome of deficiency of kidney qi and phlegm and blood stasis.
