The course of COPD can be subdivided into acute exacerbation and stable period.Most physicians and patients pay greater attention to patients with COPD with acute exacerbation,and ignore the rehabilitation and management of stable patients.Resulting in an increase in the number of exacerbations and worsening of patient's condition.At present,research on home care systems for COPD is still in its infancy.Most of the research mainly stays in the monitoring of the subject's physiological index stage,ignoring the management of the patient's entire home care process.Based on the above situation,this paper proposes a comprehensive care pathway and acute exacerbation model for chronic obstructive pulmonary disease.The purpose is to provide personalized home care plans and early warning of acute exacerbations for patients with COPD in different conditions.And this paper designs a home care system for patients with COPD.The main work in this paper has the following aspects:1.A patient-centered home comprehensive care path is proposed.The comprehensive care path is a patient-centered home care method for the purpose of promoting patient self-management.This method innovatively correlates the patient-generated data with an automatic evaluation of the severity of the patient and then sends the evaluation report to the clinician to confirm the severity.A personalized home care plan is provided for patients based on comprehensive evaluation results.Finally,through a specific case,the verification of the effectiveness of the personalized care plan generation algorithm is completed.2.A comprehensive evaluation model for assessing the severity of COPD patients was proposed.In this paper,the linear kernel support vector machine and the random forest classifier are integrated to construct an integrated classifier by means of weighted voting.So that the comprehensive evaluation of the patient's severity level can be achieved.Through experiments,the comprehensive evaluation model of COPD has been validated using 812 patient data marked by doctors,and satisfactory results have been obtained.3.An individualized early warning model for exacerbation of COPD patients was proposed.The acute exacerbation warning model triggers warnings and alarms by detecting the occurrence of events related to the exacerbation of COPD,so as to beable to intervene in a timely manner to the deterioration of the patient's condition.In this paper,the colored Petri net is utilized to reflect the dynamic changes of the alarm status over time caused by the occurrence of events related to the exacerbation.An estimation algorithm for individualized free parameter in triggering warning and alarm constraints is given.Using data collected from 20 groups of patients for two weeks,the validity of the individualized acute exacerbation warning model was verified.4.Based on the comprehensive care path and the warning model of acute exacerbation,the architecture,software architecture,data model and service flow of home care system for patients with COPD were designed.And the prototype of the home care system for patients with COPD is also presented.
