Objective:This study is for Kunshan cognition Sixth People's Hospital respiratory disease and extension service needs of COPD patients discharged after medical investigation and clinical analysis, combined with the theory of Orem self-care and health promotion model to explore and develop the application of this area of COPD patients after hospitalized in stable condition and discharged after extended care and home care process measures, to improve the stability of COPD patients healthy self-care, improving the quality of life of the population, reduce disease recurrence, reduce readmission rates.Method:This study is divided into two parts, the first part refer to the literature and expert guidance on the basis of a questionnaire designed to screen cases of COPD patients discharged after demand for disease cognition and community outreach services survey, analyze and summarize and to develop appropriate quality nursing extension service measures.
