Treatment Of Skin Defect By Stretch Regeneration With Law Of Tension Stress

Objective To investigate the clinical therapeutic effect of stretch regeneration with law of tension stress for skin defect.Methods We collected and analyzed 18 skin defect cases of the treatment with skin traction band made of sterile blood transfusion apparatus and sterile nylon strip in our hospital from March 2016 to December 2017.There were 13 males and 5 females,aged 7 ~ 68 years old,average 35 years old,with the skin defect area is about 6cm×3cm to 20 cm×5cm.A simple skin traction band was made of a sterile the blood transfusion apparatus and a number of aseptic nylon bands.After thorough debridement,it was placed on both sides of the wound margin,1 cm apart from the wound margin.The distance between the holes was 1.5cm ~ 2 cm.A simple tension band was installed along the edge of the wound.After operation according to the tension,the skin margin blood and distal limb feel,we gradually tightened the skin traction band to shrink and close the wound until the skin border fit,then interrupted suture the skin.Results 15 cases of 18 skin wound cases with skin traction band were sutured directly.2 cases of Foot trauma with infection,because of thetraction band pulled out along the skin border,we used vacuum sealing drainage(VSD)and skin graft to repair the wound.1 case of forearm skin defected with infection received skin graft treatment after traction band failure.After3~24months of follow-up,patients with skin traction band could obtain the linear scar healing,soft scar,little pigmentation,without tickle,without adjacent joint dysfunction due to skin contracture and normal pain sensation.Conclusion It is an effective,simple and economical treatment method to treat skin defect with stretch regeneration with law of tension stress and deserved to popularize.
