Objective:To investigate the postoperative adjacent vertebral body refracture status with unilateral and bilateral percutaneous vertebroplasty, and whether there's any difference among the factors affecting the refracture in two methods. Methods:According to the entry conditions,123 patients were selected, who were treated in a tertiary hospital due to OVCF(osteoporotic vertebral compression fracture)from February 2013 to February 2015.There were 43 cases under conservative treatment, and 80 cases with surgical treatment, and among these 80 cases there were 45 cases with unilateral PVP and 35 cases with bilateral PVP,and all the patients were followed up for 3 years.Firstly, compare the differences in refracture rate,VAS score of both pretherapy and 3 days after the treatment, bone mineral density, body mass index, age, the gender of both conservative treatment group and surgical treatment group. Then conduct a comparison between the unilateral puncture group and the bilateral puncture group to investigate the differences in bone mineral density, body mass index, age, gender, bone cement injection volume, intervertebral leakage rate, distribution status, vertebral height and Cobb angle change, operation time and X-ray exposure times. Results:There was no statistical significance between the conservative treatment group and the surgical treatment group in terms of bone mineral density, body mass index, age, gender, pretherapy VAS score(P>0.05),while it was statistically significant in refracture rate and the VAS score of 3 days after the treatment(P<0.05).It had no statistical significance in refracture rate, bone mineral density, body mass index, age, gender, bone cement intervertebral leakage rate, vertebral height and Cobb angle of pretherapy and post-treatment between the unilateral PVP group and bilateral PVP group(P>0.05),while it had significant difference in bone cement injection volume, distribution status, operation time and X-ray exposure times(P<0.05).Conclusion:1.The refracture rate of conservative treatment group is lower than surgical treatment group, but the effect of pain relief was relatively poor in the conservative treatment group, and the life quality of the patients was seriously reduced,so surgical treatment is recommended.2.There was no significant difference in adjacent vertebral body refracture rate of two methods after PVP.Due to the short operation time required for unilateral puncture, the pain patients suffered and the X-ray patients received were much less, and the surgical method was relatively simple, which is recommended.3.The bone cement injection volume of unilateral PVP is less than bilateral PVP,but the bone cement can be unevenly distributed easily. If uneven bone cement distribution is found during unilateral PVP operating,it is recommended to change to the bilateral puncture method.
