Randomized Controlled Research On Clinical Features,Blood Glucose Control,Quality Of Life And Therapeutic Compliance Of Type 2 Diabetic Patients In Xuguan&Yangshan Community Between 2015 To 2016

Objective To analyze the demographic data,characteristics of drug treatment,blood glucose control,quality of life and therapeutic compliance of type 2 diabetic mellitus(T2DM)patients in Xuguan & Yangshan community between 2015 and 2016.Methods We formulated the diagnostic criteria,inclusion criteria and exclusion criteria for T2 DM patients,and totally 1248 T2 DM patients in community health centers in suzhou city between 2015 and 2016 were selected,624 cases in each year.For T2 DM patients,demographic data [including age and gender,weight,body mass index(BMI),duration of diabetes,previous medical history,family history,education level,income level,and complications],the clinical information [including fasting blood glucose(FBG),2 hour postprandial blood glucose(2 h PG),glycosylated hemoglobin(Hb A1c),blood lipid indexes such as total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDLC),high density lipoprotein cholesterol(HDL-C)],types and use of hypoglycemic drugs were collected carefully.According to the pathogenic characteristics of disease,patients were further divided into primary diabetic group and occurred diabetic group.The differences of clinical data were compared in T2 DM patients selected in different years and groups.According to the final therapeutic status after one year,the T2 DM patients were divided into single oral hypoglycemic drug group,multiple oral hypoglycemic drugs group,oral hypoglycemic drug combined with insulin group and hypoglycemic drugs reduction group,and differences in BMI,HbA1 c,blood sugar indexes and blood lipid indexes were compared between the four groups.The health status scale(SF-36)was used to evaluate the quality of life before follow-up and one year after follow-up in all the patients and between primary diabetic group and occurred diabetic group.According to the level of blood glucose and blood lipid,the therapeutic compliance of patients was evaluated,which including complete compliance,partial compliance and no compliance.The total compliance rate was calculated by full compliance and partial compliance.Results In the comparison of demographic data and clinical information,there were no significant differences in T2 DM patients between 2015 and 2016(P>0.05),but between T2 DM patients in the same year,there were still some differences,such as the ratios of overweight and obese patients were significantly higher than those in normal patients(P<0.05),the most of patients were over 45 years old,the ratio of patients complicated with diabetes mellitus was significantly higher than that of patients without diabetes mellitus(P<0.05),the proportion of patients with primary diabetes was significantly lower than that of patients with occurred diabetes(P<0.05),the most common education level was junior middle school and high school level,the most common per capita monthly income level was 2 000 to 4 000 RMB,the proportion of patients with combined therapeutic drugs was significantly higher than that of patients with single drugs(P<0.05),and the most common type of hypoglycemic drugs was oral hypoglycemic drugs.In year 2015 and 2016,there were significant differences in male ratio,average weight,average BMI,average age,age distribution,family history of diabetes,smoking history,per capita monthly income,blood glucose indexes,HbA1 c and blood lipid indexes between primary diabetic group and occurred diabetic group(P<0.05),especially expressed as the male,age from 25 to less than 45 years old and income level over 4 000 RMB were the majorities in T2 DM patients in primary diabetic group.In comparison of diabetes treatment,the levels of BMI,blood glucose and blood lipid significantly improved when compared with those before treatment in year 2015,2016 and between primary diabetic group and occurred diabetic group in the same year(P<0.05).In year 2015 and 2016,the control levels of FBG and 2 h PG in multiple oral hypoglycemic drugs group and oral hypoglycemic drugs combined with insulin group were significantly lower than those in the single oral hypoglycemic drug group and oral hypoglycemic drug reduction group(P<0.05).After one year of follow-up,the scores of physiological function and mental function in T2 DM patients in year 2015 and 2016 increased significantly when compared with those before follow-up(P<0.05).In the same year,the scores of physiological function,vitality and social function of primary diabetic group were significantly lower than those of occurred diabetic group,while the score of emotional function was significantly higher than occurred diabetic group(P<0.05).In year 2015,the rate of therapeutic compliance in T2 DM patients was 79%(493/624),which was equal to 79%(493/624)in year 2016.In the same year,the rate of therapeutic compliance in primary diabetic group was significantly higher than that in occurred diabetic group(P<0.05).Conclusions The attack of type 2 diabetes mellitus in community is characterized by high body mass,elder age and family history,and it is mainly treated by oral hypoglycemic agents,and the combination of multiple drugs is recommended.The diabetes with long course has a severe influence on quality of life in the patients,so health education should be strengthened to improve the rate of therapeutic compliance and prognosis.
