| Objective:By classifying TCM patients with simple tinnitus or tinnitus with nerve deafness,we find the general clinical characteristics of tinnitus patients and the distribution rule of TCM Constitution.It is also to explore the characteristics of tinnitus in patients with different physique types and to reveal the relationship between the psychoacoustic results of tinnitus in patients with simple neurotic tinnitus or those with neurodeafness and the physique of traditional Chinese medicine(TCM).In deepening the recognition of the characteristics of tinnitus among different physique people,this paper lays a foundation for the establishment of a new individualized and accurate Chinese medicine program for the prevention and treatment of tinnitus,in order to enlighten the individualized dialectical treatment of tinnitus and provide a new way for the clinical study of tinnitus.Methods: 151 cases of simple tinnitus as the first complaint or tinnitus with neurodeafness were collected.All the patients were instructed to fill out the Self-Test Table of TCM Constitution Classification.The patients were classified according to the same TCM physique type.Filling in the tinnitus disability quantitative form,all the patients were tested with psychoacoustics of tinnitus,including tinnitus frequency,loudness,Minimum masking level(MML)and residual inhibition test.The statistical analysis of the data is carried out by using the spss22.0 statistical software.Results: 1?Among the physique types of tinnitus patients,mild constitutionaccounted for 15.89%,partial constitution accounted for 84.11%,including of qi deficiency constitution 33.11%,yangxuzhi 17.88%,yinxuzhi 10.60%,Qi-stagnation constitution 7.95%,qi deficiency plus constitution 9.27%,phlegm dampness constitution 2.65%,Blood stasis constitution 2.65%.2 ? To avoid statistical errors caused by small sample size,the correlation of Chinese medicine constitution,Tinnitus Psychoacoustics and disability quantitative form among the four groups: mild constitution,qi deficiency constitution,yangxuzhi,and yinxuzhi.(1)There was no statistical difference in tinnitus frequency matching between them in 4 groups respectively(P>0.05).(2)There was statistical difference in the mean tinnitus loudness between patients with yangxuzhi and yinxuzhi(p < 0.05).The loudness of yangxuzhi was 23.78±17.74 db and that of yinxuzhi was 39.38 ±23.01 db.(3)There was no statistical difference in the distribution of residual inhibition test between the two groups(P > 0.05).(4)There was no statisticalt difference in the distribution of masking thresholds in the 4 groups(P > 0.05).(5)There was no statistical difference in the distribution of THI score(P > 0.05).3?In terms of sex of tinnitus patients,the ratio of male to female is 2 / 3.There was no statistical difference in the sex-distribution(P > 0.05).In terms of the patient's age,The youngest age is 11 years,the maximum age is 83 years,patients between 41 and 60 years of age accounted for 46.35%;4?In terms of duration of disease in tinnitus patients,the acute accounted for 50.99%,the subacute accounted for 23.84%,the chronic accounted for 25.17%.There was no statistical difference in duration of disease(P > 0.05).5?The monaural disease accounted for 72.85%,the binaural disease accounted for 27.15%;6 ? In the results of acoustic conductance in tinnitus patients,Type A accounted for 93.37%,Type As accounted for 6.62%.7?Thepatients(50.33%)had hearing loss,the other(49.67%)had no hearing loss.There was no statistical difference in hearing loss in tinnitus patients(P<0.05).8?The patients accompanied with vertigo accounted for 72.85%,The other without vertigo accounted for 27.15%,There was statistical difference in tinnitus patients accompanied with vertigo.9 ? The tinnitus with sleep disorder accounted for75.50%,The other without leep disorder accounted for 24.50%.There was no significant difference in whether the patients associated with vertigo(P>0.05);10?The patients with emotional disorders accounted for 70.20%,The patients without emotional disorders accounted for 29.80%,There was no significant difference in whether the patients associated with emotional disorders(P>0.05);11 ? According subjective emotional disorder in patients with tinnitus,the samples were divided into four groups: non-emotional disorder group,mild group,moderate group and severe group.There was significant difference between the four groups in the THI scale scores(P<0.05);12?According to the course of tinnitus patients,the samples were divided into three groups: acute,subacute and chronic diseases.There was statistical difference between the chronic diseases group and the acute diseases group I nConscious emotion(P<0.05);There was no significant difference in THI grades and scores between the three groups(P>0.05).Conclusions: 1?The physical types of tinnitus patients were mainly partial constitution,among them,the most common type is asthenia constitution accompanied by vertigo and sleep disorders.Dialectical therapy for Tinnitus in Chinese Medicine should be based on asthenia constitution.It shows that the disease of tinnitus,sleep disorder and vertigo can be used for individual treatment.2?The matching loudness of tinnitus in patients with yangxuzhi islower than yinxuzhi.It shows that the low clinical matching loudness may be related to yangxuzhi.3 ? It is suggested that clinical psychoacoustics test of tinnitus should be added as a necessary examination item for tinnitus patients to help judge their physique types according to the results of psychoacoustics of tinnitus.4?The longer the course of tinnitus,the less the negative emotion feeling.However,it is not obvious in the THI score.The level of self-emotional feelings of tinnitus patients is basically the same as the THI score.The contents of the THI scale should be further enriched.In the clinical examination of tinnitus patients,it is necessary to apply it to the assessment of tinnitus,combined with the use of their own emotions,so that the patients with various physique can be treated with Tinnitus retraining therapy to assist in clinical diagnosis and treatment. |
