Objective:To analyze the clinical effect of Cognitive Behaviora Therapy(CBT) and Tinnitus Retraining Therapy(TRT) in the management of patients suffering from tinnitus.Methods: 1. To select the tinnitus patients who visit in the First Affiliated Yijishan Hospital of Wannan Medical College ENT outpatient from September 2013 to March 2015 and provide audiological testing and related physical check. 2. To collect the patient’s general situation by questionnaire(THI and TRI)and assess the condition.3. All patients with tinnitus were treated with TRT and CBT. The TRT consisted of three strategies : tinnitus masking with low level and broad band noise; diversion of the attention to other things; psychological counseling and therapy.The CBT consisted of three strategies: cognitive restructuring of the thoughts;coping-skills therapies;problem-solving therapies.They were aimed to provide relevant knowledge for adaptation and habit.4. Efficacy was evaluated by Tinnitus Handicap Inventory( THI) and Restless Inventory before and 6th month after treatment.To analyze the relationship between gendes,ages,diseased parts,degree of hearing of loss and tinnitus severity.Results: 1. The THI between 6th month after treatment(25.50±16.90) point and prior treatment( 43.31±19.90) point was statistically significant( P <0.05).2. The tinnitus handicap degree 2 and /or less,and Restless degree 2 and /or less were evaluated as mild tinnitus,while tinnitus handicap degree 3 and /or more,and Restless degree 5 and /or more were evaluated as severe tinnitus.Restless degree 3 and 4 were evaluated as moderate tinnitus.According to the tinnitus handicap evaluation,there were respectively 54( 51.92%,54/104)before treatment, 84( 80.77%,84/104)after 6 month of treatment.while the severe tinnitus case number were 50( 48.08%,50 /104) before treatment,20(19.23%,20/104) after 6 month of treatment with significant difference( χ2 = 19.38,P< 0.05).According to the Restless inventory evaluation,there were respectively 43( 41.35%,43/104)) before treatment, 88( 84.62%,88/104)after 6 month of treatment in mild tinnitus cases while the severe tinnitus case number were 5( 4.81%,5/104) before treatment,2( 1.92%,2/104)after 6 month of treatment,thoderate e mtinnitus were 56(53.85%,56/104)) before treatment,14(13.46%,14/104) after 6 month of treatment,respectively,with significant difference( χ2 = 41.94,P < 0.05).3. No significant correlation was found between genders,ages,hearing loss,diseased parts and tinnitus severity through statistical analysis.Conclusion: Cognitive Behavioral Therapy(CBT) and Tinnitus Retraining Therapy(TRT)is expected to be an comprehensive intervention in the management of patients suffering from tinnitus.
