ObjectionTo detect the expression of acidic mammalian chitinase(AMCase)and its downstream molecules eotaxin-3 and IL-13 in human chronic rhinosinusitis.Materials and Methods1.20 patients with simple maxillary sinus cyst were treated with uncinate process mucosa as control group,30 cases of chronic sinusitis with nasal polyps(CRSwNP)patients with nasal polyp tissue and 30 cases of chronic sinusitis without nasal polyps(CRSsNP)sinus mucosal tissue were served as the observation group.2.The expression of AMCase,eotaxin-3 and IL-13 in the nasal mucosa of the control group and the observation group were detected by immunohistochemistry and polymerase chain reaction(PCR).All patients underwent preoperative blood tests for allergens,history of asthma,nasal endoscopy,CT scans of the sinuses(crown + axial + sagittal),blood eosinophil counts,blood eosinophil percentages,and IgE level determination.3.SPSS 22.0 software is used to analyze data.Results1.There was no significant difference in the distribution of gender,age and clinical stage between CRSwNP group,CRSsNP group and control group(P>0.05).2.The proportion of patients with a history of AR or asthma was higher in the CRSwNP group(66.7%,25%)and CRSsNP group(53.3%,14.3%)than in the control group(45%,5%),but the differences were not statistically significant(P>0.05).The comparison of allergic factors in CRSwNP and CRSsNP groups of different clinical stages revealed that the incidence of AR,the incidence of asthma,and the positive rate of EOS were increased in the CRSwNP and CRSsNP groups as the stage increased,especially the CRSwNP group was more pronounced in patients with various stages of AR,asthma history,and EOS positive rates(P<0.05).3.Compared with the control group,the CRSwNP group,CRSsNP group and the control group had higher eosinophil counts and eosinophil percentages than the control group,and the difference was statistically significant(P<0.05);There was a positive correlation between blood EOS counts and percentages in patients with CRSwNP and CRSsNP(r=0.64 0.54,P<0.05).4.The total serum IgE levels in the CRSwNP ?CRSsNP and control groups of patients were(4.60 ± 0.63?4.43 ± 1.30 ? 4.00 ± 0.67),respectively.The total IgE levels in the first two groups were higher than those in the control group(P=0.02<0.05).However,there were no statistically significant differences between the CRSwNP group and the CRSsNP group(P>0.05).5.Immunohistochemistry and PCR results showed that the expression rates of AMCase,eotaxin-3,and 1L-13 in the CRSwNP group and CRSsNP group were significantly higher than those in the control group(P<0.05);they were expressed.The rate of the CRSwNP group was higher than that in the CRSsNP group(P<0.05),and there was a positive correlation between AMCase,eotaxin-3 and 1L-13(r=0.56 0.62 0.48,P<0.05).Conclusion:1.The blood eosinophil count and eosinophil percentage in CRS patients were higher than those in the control group,and they were positively correlated and may be related to the overexpression of eotaxin-3.2.Both AMCase and its downstream eotaxin-3 and IL-13 were detected in both CRSwNP,CRSsNP and control tissues by immunohistochemistry and RT-PCR.The expression levels of the three groups were higher than the control group.The group showed that there was a certain amount of physiological expression in the normal tissues,and the expression increased when stimulated by certain stimuli.3.The abnormally increased expression of AMCase,eotaxin-3 and 1L-13 may be related to the occurrence and development of CRSwNP and CRSsNP diseases,and their overexpression in CRSwNP may be related to the abnormal proliferation and apoptosis of nasal polyps.4.The expression of AMCase,eotaxin-3and 1L-13 were positively correlated in CRS.The expression levels of AMCase,eotaxin-3 and 1L-13 mRNA were related to the pathogenesis of CRS.The three promoted each other.It may serve as a reference indicator for the prognosis of CRS disease.5.The search for blockers or antagonists of AMCase,eotaxin-3,and 1L-13 related factors may be a way to treat CRS.
