Objective To discussing the relationship of allergen IgE concentration gradientand Acidophilic cell infiltration and chronic sinusitis and polyps.Methods The data of152CRS or nasal polyps surgery patients,including88cases chronic Rhinosinusitis with Lpolys (CRSwNp) and64cases chronicRhrinosinusitis without Lpolys (CRSsNp), were collected from May2011to October2012. The patients’ acidophil of blood was detected by the SYSMEX XE-2100Dfullyautomatic hematology analyzer and serum special IgE was analyzed by Allergy Screenallergen detection system. Meantime discuss the relationship of allergen IgEconcentration gradient, allergic factors, acidophilic cell infiltration and nasal polypthrough patients’ clinical data. Results1.Allergen IgE classification of CRSwNpfocused on Ⅱlevel, but the allergen IgE classification of CRSsNp focused on Ⅰlevel.There was no statistically significant difference between them(P=0.365>0.05);2.Although there were88cases patients with allergic disease in the CRSwNP group and64cases in the CRSsNP group, no statistically significant difference between allergicdisease and the occurrence of nasal polyps(P>0.05);3. The acidophilic cell percentagein the blood of CRSwNP patients apparently higher than CRSsNP group (F=4.337,P=0.039;t=3.315,P=0.001,95%CI3.90-5.26). There was statistically significantdifference between them (P<0.05);4. In spite of the allergens positive detection rate ofCRSwNP and CRSsNP group were53.41%and51.56%respectively, but there was nostatistically significant difference between them (P>0.05). The allergen composition isgiven priority to with aspiration (90/115,78.26%). Interestingly, Household dust mitesare the most common, followed by penicillium notatum and cypress/elm/Chineseparasol;5. The majority of allergen was mixed allergens in the CRSwNP group, andwith the increase of concentration gradient of the allergen, mixed allergens in patientswith allergic ratio showed a trend of increase(X2=8.595,P=0.015). Conclusions1.It was involved multifactor in the formation of mechanism of nasalpolyps in patients with CRSwNP. So could not simply to see sIgE, serum total IgE,acidophil blood test results as a judge index.2. Compared with CRSwNP and CRSsNP,the serum total IgE concentration mainly focused on Ⅰlevel in the CRSsNP group, butmainly focused on Ⅱlevel in the CRSwNP group(Z=0.906,P=0.365). There was nostatistical difference of concentration gradient distribution between them (P>0.05). Itpointed out serum total IgE could not completely as judge index of the CRSwNPoccurred;3. There was a close relationship between chronic sinusitis with polyp andwith allergic diseases and acidophilic cell infiltration;4. The present study provided thebasis for allergen screening methods to choose. As this experimental reagent had strongspecificity and weak sensitivity and the presence of allergens had certain correlationwith sinusitis, so it can’t as the diagnostic basis of chronic sinusitis.
