| Objective: To analyze the correlation of hematocrit(HCT),fibrinogen(Fib),D-dimer(DD)and carotid plaque formation,and to investigate the association of carotid plaque formation with coronary heart disease and cerebral infarction.Methods: The clinical data of 283 patients who underwent carotid ultrasonography in our hospital from January 2013 to June 2017 were analyzed retrospectively,the patients were divided into plaque group(n=162)and non plaque group(n=121).The HCT,mean corpuscular volume(MCV),Fib,DD,fibrinogen degradation products(FDP),blood pressure and biochemistry indexes were detected and compared between the two groups.Then the independent risk factors of carotid plaque formation were screened by logistic regression analysis,the non-independent risk factors were analyzed by spearman correlation analysis,and to explore the correlation between these indexes and carotid plaque formation.Of all the 283 enrolled patients,149 accepted coronary angiography(CAG)examination and 128 underwent head CT or MRI examination at the same time.According to CAG,head CT or MRI results,we analyzed whether there was a difference in the incidence of coronary heart disease and cerebral infarction between the plaque and non-plaque group.Results: 1.In plaque group,the levels of Fib,DD,MCV,FDP,SBP,PP,FBG,CRE,BUN and BUA were significantly higher,while HCT and diastolic blood pressure(DBP)were significantly lower than those in non plaque group(P<0.05).2.In logistic multivariate regression analysis,elderly(age≥60),increased Fib(Fib>4.0 g/L),FBG(FBG>6.11 mmol/L)and PP(PP≥60 mm Hg)were all shown as the independent risk factors for carotid plaques formation.3.Spearman correlation analysis showed that carotid plaque formation was positively related to DD,FDP,MCV,SBP and BUN,whereas negatively related to HCT and DBP(P<0.05).4.In the 149 patients who accepted CAG examination,the positive rate of CAG and the Gensini score were significantly higher in plaque group,and the rate of carotid plaque formation in multivessel disease group was higher than that in single vessel disease group and non-coronary disease group(P<0.01),however,there was no significant difference between single vessel disease group and non-coronary disease group(P>0.05).Spearman correlation analysis showed that there was a significant positive correlation between the Gensini score and carotid plaque formation(r=0.385,P=0.000).5.In the 128 patients who underwent head CT or MRI examination,the positive rate of cerebral infarction in plaque group was significantly higher than it in non plaque group(P<0.01).And spearman correlation analysis showed that there was a positive correlation between the incidence of cerebral infarction and carotid plaque formation(r=0.243,P=0.006).Conclusion: 1.Many risk factors,such as age,HCT,MCV,Fib,DD,FBG,SBP and PP,were involved in the formation of carotid plaques,of which elderly(age≥60),increased Fib(Fib>4.0 g/L),FBG(FBG>6.11 mmol/L)and PP(PP≥60 mm Hg)were all independent risk factors of carotid plaque formation.2.Carotid plaque formation was significantly correlated with coronary heart disease and cerebral infarction. |
