Objective1.Evaluation of the Impact of Competency-based Education on care ability?quality of life?QO-L?and Caregiver Strain Index?CSI?of two groups of caregivers.2.Explore the feasibility and effectiveness of implementing empowerment-based education to improve the care ability and quality of life of the spouse caregivers of stroke patients and reduce the burden of Caregiver Strain Index.at admission,discharge,1thmonth,3thmonth after discharge.Methods.The study is divided into two parts1.Firstly,qualitative interviews were conducted to conduct semi-structured interviews with 18 stroke caregivers to understand their psychological response to the patient's onset,the need for healthy education and the cognition and view of traditional health education.2.secondly,120 patients with stroke and spouses caregivers from the first affiliated hospital of Zheng Zhou university were selected in this study,From November 2016 to June 2017.The spouse caregiver in November 2016 to February were control group,from march 2017 to June 2017 were the experimental group.The control group has routine health education.It mainly includes:basic nursing,specialist nursing,complication nursing,the guidance of education.Empowerment-based education consist of group discussion and individual education,five step empowerment approach was used.the instruments include general situation questionnaire,Comprehensive care ability scale?CCAS?,Care Quality of Life?Care-QL?,Caregiver Strain Index?CSI?,readmission rate.Data were collected at admission,discharge,1thh month,3thh month after discharge.Data was analyzed with SPSS19.0,The results indicated a satistically significant difference?P<0.05?,at two-tailed significant level 0.05.Result1.The study was conducted by 120 caregivers.The control group completed 56cases with an effective rate of 93.3%.The intervention group completed 57 cases with an effective rate of 95%.2.There is no significant difference between the two groups of patients and their spouse caregivers in the baseline date?P>0.05?.3.There was no statistically significant difference in CCAS?Care-QL?CSI.There is statistically significant difference?P<0.05?in score of CCAS and each dimension at 1th month,3th month after discharge.4.There was no statistically significant difference between the groups in Care-QL?and Care burden before the intervention?P>0.05?.There were no statistically significant differences in triglycerides,total cholesterol,high-density lipoprotein,and low density lipoprotein?P>0.05?in the two groups of patients at 1th month,3th month after discharge.Conclusion1.The empowerment-based education is an effective intervention,it can constantly significantly improve the care ability?quality of life about spouse caregivers,and it can educes the care burden at 1thmonth,3thmonth after discharge.2.It suggests that we should take the initiative measures to develop caregivers itself to take care of the potential and help caregivers to discover and develop self-management responsibility ability,making management choices and decisions actively,improve the prognosis of patients and reduce reentry rate.
