Objective To explore the effect of empowerment-based education on nutritional risk, self-efficacy, self-care ability and nutritional status of hospitalized patients with chronic liver failure, and to evaluate the effect of intervention.Methods Selecting February-2015 to July-2015 in Zhengzhou City of a tertiary general hospital infection department of 68 cases of chronic liver failure patients, at the first three months we selected 34 cases as the control group, they received routine clinical health education for liver disease patients; And at the last three months we selected other 34 cases as the intervention group, they received empowerment-based education. The instruments include demographic questionnaire, NRS-2002, Child-Pugh scale Self-Efficacy Scale for Chronic, Self-Management Scale and nutrition indexes related with physical and biochemical examination. The score of NRS-2002, Self-Efficacy Scale for Chronic, Self-Management Scale and nutrition indexes related with physical and biochemical examination of two groups were compared three times(before intervention and after 1 month and 3 months). Processing the data with SPSS 17.0 statistical software, and the level of alpha=0.05 was tested. Statistical results using s. Statistical methods using two independent samples t test, chi square test, repeated measurement analysis of variance.Results 1. The study included 68 patients, of which the intervention group(34 cases), the control group(34 cases), in the intervention process 9 patients were lost for the reasons of liver transplantation, infection, illness, death and automatically exit, the rate of loss sample was 13.2%. 59 patients completed the whole process of intervention, 30 cases in the empowerment group and 29 cases in the control group. The baseline data of the two groups were balanced before the intervention, and there was no statistical significance(P>0.05); 2. Before the intervention compared the scores of NRS-2002 of two groups. : the NRS-2002 score of intervention groups was(3.57± 0.94), while the control group was(3.31 ±0.97), the statistical analysis showed that there was not statistically significant difference between two groups(P > 0.05). we compared the NRS-2002 scores of two groups after 1 month and 3 months, the result shows that the scores of intervention group decreased greatly(P < 0.05); 3. Before the intervention the scores of self-efficacy Scale for Chronic of two groups were in middle level.The scores of self-efficacy of CLF patients of the intervention group significantly increased compared with the scores of the control group after repeated measurement analysis of variance(P < 0.05); 4. Before the intervention the scores of Self-Management Scale of two groups were in middle level.The scores of Self-management of CLF patients scores of the intervention group significantly increased compared with the scores of the control group after repeated measurement analysis of variance(P < 0.05); 5. Before the intervention the nutritional indexes related with physical and biochemical examination of two groups had no statistical difference(P > 0.05); After 1month and 3 months, TSH, AMC, ALB, PA in the intervention group significantly increased compared with the control group after repeated measurement analysis of variance(P < 0.05), but the BMI index of two groups had no statistical difference(P > 0.05).Conclusion 1. Nutritional risk exists in patients with CLF, medical staffs should pay more attention to them and give them timely treatment; 2. Empowerment-based education can effectively reduce the score of NRS-2002 of CLF, and it also can improve the self-efficacy and Self-management ability of patients with CLF; 3. Empowerment-based education can not effect the BMI index of patients with CLF, but in improving TSF, AMC, ALB, PA, the effect is obviously.
