Objective: To describe the status quo of care burden and the level of empowerment ability of the main caregivers in the family of type 2 diabetic patients,to explore the influencing factors of care burden and empowerment ability and the correlation between them.So as to provide scientific basis and new ideas for improving caregivers' empowerment ability,reducing care burden,promoting caregivers' physical and mental health,and carrying out targeted nursing intervention.Methods: This study is a cross-sectional design.From March 2019 to June 2019,164 patients with type 2 diabetes mellitus and their main caregivers in the Department of endocrinology of grade-A tertiary hospital in Luzhou City were investigated by questionnaire.The general information questionnaire,Zarit Burden Interview(ZBI),Main Caregivers' Empowerment Measurement(MCEM),and the Social Support Rating Scale(SSRS)were used to survey the primary caregivers.The patient general information questionnaire and the Modified Barthel Index(MBI)were used to survey patients.Use EpiData 3.1 to build database and SPSS 20.0 to analyze data.Results:1 The current situation and influencing factors of care burden of the main family caregivers of type 2 diabetic patients1.1 The total care burden of the main caregivers was 32.82 ± 11.91,personal burden 19.75 ± 5.75,and responsibility burden 9.04 ± 4.78,including 24 cases of no burden(14.6%),85 cases of light burden(51.8%),51 cases of moderate burden(31.2%),and 4 cases of heavy burden(2.4%).1.2 The results of univariate analysis showed that the relationship among caregiver's residence,education level,occupation,period of care,average daily care time,total monthly family income,patient's gender,age,residence,education level,monthly medical expenses,payment form of medical expenses,the number of hospitalization due to diabetes,insulin injection,patient's ability of daily living activities and social support with total burden score was statistically significant(P < 0.05).1.3 The results of multivariate analysis showed that there were significant relationship among caregiver's occupation,average daily care time,social support,monthly medical expenses and activities of daily life with caregiver's care burden(P < 0.05).2 The empowerment level and influencing factors of the main family caregivers of type 2 diabetic patients2.1 The total score of the main caregivers' empowerment ability was 154.79 ± 10.51,the highest score was "relationship with the caregiver" 20.81 ± 2.32,and the lowest score was "knowledge and skills of care" 7.27 ± 1.62.2.2 The results of univariate analysis showed that the relationship among age,residence,education level,occupation,average daily care time,total family income,gender,residence,education level,monthly medical expenses,payment form of medical expenses,the number of hospitalizations due to diabetes mellitus,the ability of daily living activities of the patients and social support with the ability of empowerment of the caregivers were unified Statistical significance(P < 0.05).2.3 The results of multi factor analysis showed that the relationship among social support,average daily care time with caregivers' empowerment was statistically significant(P < 0.05).3 Pearson correlation analysis showed that the care burden of the main caregivers in the family of type 2 diabetic patients was negatively correlated with their empowerment.Conclusion:1 The main family caregivers of type 2 diabetic patients generally have light and medium care burden.The influencing factors of care burden of caregivers include: caregiver occupation,daily average care time,social support,monthly medical expenses and activities of daily life.2 The main caregivers' empowerment in the family of type 2 diabetic patients is at the middle level.The influencing factors of the empowering ability of the caregivers include: social support and the average daily care time.3 The heavier the care burden of the primary caregiver in the family with type 2 diabetes,the lower the level of caregiver empowerment.
