Clinical Observation On Astragalus Tetrandrae Decoction Plus Ephedra In Treatment Of Chronic Cardiac Failure With Water Overflowing Caused By Insufficiency Of Yang

Objective:To observe the improvement of symptoms before and after treatment in patients with chronic heart failure with yang-deficiency water,as well as the improvement of cardiac function parameters,NT-pro BNP and6-minute walking distance test,it provides reference for clinical application of this drug.Methods:From April 2017 to December 2017,In the first affiliated hospital of guangxi university of traditional Chinese medicine,there were 60 cases of patients with chronic heart failure,SPSS21.0 was randomly divided into the western medicine group(30 cases)and the Astragalus Tetrandrae Decoction plus Ephedra group(30 cases).The western medicine group was treated with perindopril tert-butylamine tablet/urbesartan tablets,successively release of succinate metoprolol tablets,furosemide tablets and digoxin tablets,on the basis of the use of Chinese medicine in the western medicine group,the Astragalus Tetrandrae Decoction plus Ephedra group were added with Chinese traditional medicine,the course of treatment is one month,by comparing the parameters of cardiac function parameters,NT-pro BNP and6-minute walking distance test before and after treatment,to evaluate the curative effect of traditional Chinese medicine Astragalus Tetrandrae Decoction plus Ephedra in the treatment of chronic heart failure.Results:Before the treatment,two groups between sex,age,course,basic disease,cardiac function classification,symptoms of traditional Chinesemedicine,Lee's heart failure points,NT-pro BNP,left ventricular ejection fraction,6 min walking distance results had no statistical significance(P>0.05),comparable.After 1 month treatment,two groups of cases does not appear to eliminate,fall off and suspended phenomenon,60 cases of patients for clinical observation,in the aspect of TCM symptoms,compared with western medicine group,palpitations,shortness of breath,fatigue weakness,limb edema,chest tightness(pain),asthma,less urine could improve the cold,and cold limbs,abdominal distention,statistically significant difference(P<0.05),and spontaneous sweating,night sweats,dry mouth,cough,cough up phlegm,dysphoria,no statistical significance(P>0.05);In TCM syndromes,the total effective rate of the western medicine group was83.3%,and the total effective rate was 86.7%,and the comparison between the two groups was statistically significant(P<0.05).In the case of Lee's heart failure score,the total effective rate of the western medicine group was73.3%,and the total effective rate of the control group was 83.3%,and the comparison between the two groups was statistically significant(P<0.05);The symptoms of heart failure were integral,the western medicine group(6.70±2.575)and the Astragalus Tetrandrae Decoction plus Ephedra group(5.23±2.300),the comparison between the two groups was statistically significant(P<0.05);In terms of cardiac function classification,the improvement of Astragalus Tetrandrae Decoction plus Ephedra group was better than that of western medicine group,and the difference was statistically significant(P<0.05);In the left ventricular ejection fraction(LVEF%),there was no statistically significant difference between the two groups(P>0.05);In terms of NT-pro BNP index,the western medicine group(337.50±112.610pg/L),the Astragalus Tetrandrae Decoction plus Ephedra group(281.23±100.853pg/L),the differences between the two groups werestatistically significant(P<0.05);In the 6-minute walking distance,the western medicine group(407.43±63.540m),the Astragalus Tetrandrae Decoction plus Ephedra group(444.20±75.390m),the comparison between the two groups was statistically significant(P<0.05).Conclusion:1.Astragalus Tetrandrae Decoction plus Ephedra can improve the Yang deficiency water generic syndromes of chronic heart failure,heart palpitations,shortness of breath,fatigue weakness of TCM symptoms,limb edema,chest tightness(pain),asthma,less urine to cold,and cold limbs,abdominal distension has good improvement effect;2.It can effectively improve the cardiac function of chronic heart failure in Yang deficiency water;3.Out the Astragalus Tetrandrae Decoction plus Ephedra on left ventricular ejection fraction(LVEF%)improvement is not obvious;4.It can effectively reduce the NT-proBNP level of chronic heart failure in yang-deficiency water;5.It can effectively improve the 6min walking distance of chronic heart failure in yang-deficiency water.
