Clinical Research On Bridge Combined Fixation System (BCFS) For The Treatment Of Clavicular Fracture

Objective: Through the collection of the bridge combined fixation system(BCFS)for the treatment of clavicle fracture cases,we can observe and compare the different curative effects of the bridge combined fixation system and locking plate for the treatment of clavicular fracture.At last,we can evaluate the efficacy and safety of BCFS for clavicle fracture and explore better ways of operative treatment of clavicular fracture.Methods: Through the retrospective analysis of 66 clavicle fracture cases of surgical treatment from January,2013 to October,2016(thirty-three patients were treated with bridge combined fixation system,and the other thirty-three patients were treated with locking plate),we can compare the curative effects of them.In BCFS group,there were 19 males and 14 females.They were from 25 to 57 years old.The average age was 41.6±16.4 years old.There were 11 cases of accident injury,14 cases of fall accident from high place and 8 cases of impact injury.According to the different positions of fracture,there were 6 cases of internal 1/3 fractures,10 cases of middle 1/3 fracture and 17 cases of distal 1/3 fracture.According to the different fracture shapes,there were 23 cases of simple oblique fracture and 10 cases of comminuted fracture.In the locking plate group,there were 21 males and 12 females.They were from 25 to 55 years old.The average age was 40.4±15.2 years old.There were 13 cases of accident injury,11 cases of fall accident from high place and 9 cases of impact injury.According to the different positions of fracture,there were 5 cases of internal 1/3 fractures,14 cases of middle 1/3 fracture and 14 cases of distal 1/3 fracture.According to the different fracture shapes,there were 21 cases of simple oblique fracture and 12 cases of comminuted fracture.All cases were fresh fractures with no major vascular,nerve damage and serious complications.The clinical diagnosis,preoperative examination and assessment were performed to determine the operation time.The operation time was 2-5 days after injury,with an average of 3 days.The operative internal fixation methods were divided into two groups: bridging combined fixation system and locking plate.All patients in this study were followed up after operation.Statistical analysis was conducted on the genders,ages,causes of injury,fracture shapes and other aspects of the two groups.Two groups were observed and compared the amount,intraoperative hemorrhage,operation time,fracture healing time,shoulder Neer score,and postoperative complications if there were differences.To evaluate the efficacy of the two groups and explore the better way to treat clavicular fracture.Results: The differences of the patients' genders,ages,causes of injury,fracture shapes and other aspects of general data were statistically analyzed,and the results showed no statistical significance(P>0.05),It has comparability.The two groups were followed up for 6~12 months,with an average of 8 months.No infection was found after operation in two groups.No internal fixation loosening and fracture occurred in the fracture healing of the patients with bridge combined fixation system.One case of locking plate fractured and delayed healing occurred in the locking plate group.Compared to the locking plate group,BCFS group showed better performance in operation time,intraoperative hemorrhage and time of fracture healing.The differences among them had statistically significance.The Neer score of BCFS group was slightly higher than that of the locking plate group.But there was no statistical significance in the difference of two group's Neer score.Conclusions: The effect of treating clavicular fracture with bridge combined fixation system(BCFS)is similar to that of locking plate.However,the bridge combined fixation system has more advantages compared with the locking plate in the treatment of clavicular fracture.It can reduce the damage to the soft tissue and the peeling of the periosteum.BCFS avoids the stress concentration and stress shielding caused by internal fixation of locking plate,and it also reduces the risk of internal fixation fracture.The combination of the connecting rod and the connection block is flexible,which makes personalized treatment of the operation.We find that it is suitable for clinical treatment of clavicular fracture.
