The standardization training of residents was initiated by the Peking Union Medical College Hospital in China in 1921.For the next few decades,some tertiary hospitals also explored the standardization training for residents.In 1993,the Ministry of Health issued the "Trial Implementation of standardization training for clinical residents" in order to improve the quality of clinicians and ensure the health of patients.However,due to the imperfect system,there are still many problems in training for residents.Until 2009,as the important content of "new medical reform",the standardization training system of residents in China entered a new stage.In 2014,the standardization training management method of residents(Trial Implementation)suggested that the standardization training of residents was the core of cultivating post competency,and the training direction of residents was established.Although some studies have been devoted to the establishment of an evaluation index system for residents' competency,there is no clear definition of competency for residents in China.ObjectiveOn the basis of consulting a large number of domestic and foreign literature on training of residents,this study preliminarily constructed a competency evaluation index system for residents.The Delphi expert consultation method is used to improve the evaluation index system,and the weight of each index is determined by AHP method.Then according to the index system,we conducted a questionnaire survey to evaluate the competency of residents,the effectiveness and deficiency of resident training.To explore the influencing factors for further improvement of the hospital residents' training,and to provide evaluation tools of the selection and training for later residents.MethodsFirst,this study uses the literature review method,the Delphi expert consultation method and the analytic hierarchy process to determine the evaluation index system of the competency of the resident physician,and uses the Yaaph software to determine the weight of the index.Then the index system was applied to evaluate the competency of residents in a hospital in Shandong Province,and the data were analyzed and analyzed by SPSS22.0.T test,ANOVA are used to analysis the factors that influence residents post competency and the factors on satisfaction of standardization training.Main results?The evaluation index system of competency for residents is constructed,which contains 7 first level indicators(the mastery and application of medicine and related knowledge,clinical skills,communication skills,basic public health services,management and team cooperation,academic research and learning ability,and core values and professionalism)and the 55 two level indicators,and the weight of first level indicators are 0.19,0.21,0.17,0.08,0.12,0.10 and 0.13 respectively.The authoritative degree of experts is 0.872.After two rounds of expert consultation,the coordination coefficient of the first level index is 0.305,and it indicating that the evaluation index is desirable.?This evaluation index system is used to evaluate the post competency of a resident in a hospital in Shandong province.It is found that the score of the post competency of the resident is4.07±0.17and the level is better,but the score of the basic public health service ability is 3.77±0.77,which needs to be strengthened.?A single factor analysis of age,education,identity traits and operation opportunities shows that all above factors contribute to the post competency of residents.?The opportunity of operation is the factor that influences the satisfaction of residents to standardized training.?Qualitative interviews found that learning enthusiasm of residents is also an important factor affecting the effectiveness of standardized training for residents.ConclusionThe evaluation index of post competency of residents in this study is scientific and effective,and we can get the true level of the resident physician.It can provide a reference for the selection,training,assessment and evaluation of the residents in the future,and also can provide a new tool for the sustainable development of health care staff.SuggestionsEstablish a talent training mechanism based on the competency index system of residents:improve the selection of teachers and rewards and punishments,and strengthen the building of teachers' team;improve training system,strengthen supervision and ensure the quality of training:enriching training contents,innovating training methods,improving the comprehensive quality and learning enthusiasm of residents comprehensively;establish an objective and quantifiable evaluation mechanism and innovate incentives.
