Objectives:To investigate the breastfeeding status and duration of the postpartum nurses in third-grade class-A hospitals, analyze the influence factors of breastfeeding duration of the postpartum nurses so as to provide the relevant countermeasures and suggestions for the postpartum nurses to extend the breastfeeding duration. Methods:Adopting a convenient sampling method to select postpartum nurses from four third-grade class-A hospitals in Shandong Province, using general data questionnaire,Breastfeeding Family Support Questionnaire,Breastfeeding Knowledge Questionnaire, and the Chinese version of the IOWA Infant Feeding Attitude Questionnaire to investigate postpartum nurse who match the inclusion criteria.SPSS 20.0 was used for data entry and analysis. The analysis included statistical description,chi-square test, and variance analysis.Kaplan-Meier method was used to plot the survival curve and analysis the single factor.Cox proportional hazards model was used for multivariate analysis. Results:A total of 664 questionnaires were distributed,635 valid questionnaires were returned, effective recovery rate of 95%.Detailed results as follows:1.The exclusive breastfeeding rate of nurses in the first 6 months after parturition was 28.98%.The median of breastfeeding duration was 12 months.2.The median of postpartum nurses returning to work was 5 months, the median begin to night shifts was 12 months.3.The average score of family support for breastfeeding nurses was(3.13±0.52),breastfeeding knowledge scores averaged(14.16±2.26).The scores of infant feeding attitudes of nurses returning from postpartum averaged(65.31 ±6.5).4.The Kaplan-Meier method of single factor analysis showed that there was no significant difference in the monthly income, educational backgrounds, professional titles, positions, and the time of maternity leave(P>0.05),but different in parity times, night shift starting times, time of supplementary food supplements, time of arrivaling the hospital, whether have the milk storage conditions in working department or not, feeding method of nurses after start to work during the suckling period, whether insist to breastfeeding in the first 6 months after parturition were statistically significant(P<0.05).5.The stratified, stepwise Cox proportional hazards model analysis showed that the independent factors affecting the duration of breastfeeding for postpartum nurses includes whether they could return to home or not during suckling period and when the night shift starts.Conclusions:1.The rate of exclusive breastfeeding in the first 6 months and the breastfeeding duration for postpartum nurses are remain to be improved.2.The postpartum nurses have adequate family support and sufficient breastfeeding knowledge, the feeding attitudes of them tend to breastfeeding, all of them still need to be improved.3.The postpartum nurses’ breastfeeding duration who have two births, insist exclusive breastfeeding in the first 6 months, can return home during the suckling period, add supplementary foods later than 5 months, distance between workplace and home less than 1 hour, and night shift begins later than 12 months are relative longer.4.Whether or not can return home during the suckling period and the start time of night shifts are independent predict factors of the duration of breastfeeding.Nurses are able to return home during the suckling period will have longer breastfeeding duration than who are milk in the department. The later the night shift begins, the longer the duration of breastfeeding continues.
