Purpose:3.0T high-resolution magnetic resonance imaging technique was used to image the patients with capsular warning syndrome(CWS)and the control group.The intravascular conditions of middle cerebral artery(MCA)were compared and analyzed between the two groups.To explore the pathogenesis and the risk of cerebral infarction of capsular warning syndrome.Methods:A retrospective study of patients with transient ischemic attack(TIA)in accordance with the diagnostic criteria of CWS in our hospital was conducted as the experimental group.According to the results of brain diffusion weighted imaging within one week of onset,the patients were divided into two groups: cerebral infarction group and non-cerebral infarction group.The corresponding routine TIA and cerebral infarction patients were selected as the control group.Performance of high resolution magnetic resonance angiography were compared between the two groups.Measuring and calculating related data inclucle analyze and compare the characteristics of atherosclerotic wall,plaque load,plaque distribution and degree of enhancement of middle cerebral artery(MCA)in patients with and without infarction in the experimental group,as well as between the experimental group and the control group.Results:1.A total of 51 patients were included in the study group,including 13 patients with infarction,8 patients without infarction,30 patients in control group,10 patients with routine TIA and 20 patients with routine cerebral infarction.There was no statistical difference in clinical baseline data and risk factors of cerebrovascular disease between the two groups(p>0.05).2.In both groups,patients had definite atherosclerotic plaques in the middle cerebral artery on high resolution nuclear magnetic angiography.There are differences in the proportion of patches,the distribution of plaque location and the degree of enhancement.3.The positive rate of plaque in the experimental group with infarction was higher than that in the control group(p<0.05),but there was no significant difference between the experimental group and the control group in the patients without infarction.4.In 21 patients with CWS,the positive rate of plaque in infarction group was higher than that in non-infarction group(92.31% vs 62.50%,p=0.041).The plaque load infarction group was larger than that non-infarct group [(32.1±2.9)% versus(17.9±0.4)%;t=2.836,p=0.031].Plaque enhancement rate in infarction group was significantly higher than that in non-infarction group(p<0.05).The degree of vascular stenosis in infarction group is greater than in non-infarct group [(34.5±8.7)% versus(16.7±4.3)%;t=3.970,p=0.028].Plaque distribution in anterior inferior wall infarction group was more than in non-infarction group(p<0.05).Conclusion:1.The pathogenesis of CWS is associated with arteriosclerotic plaque,and the risk of stroke progression is related to the location of plaque distribution and stability.2.High resolution nuclear magnetic angiography can be used as an effective observation method for wall arteriosclerosis to investigate the pathogenesis of CWS,and it is of great significance to evaluate the condition of the disease and guide the relevant treatment.
