Objective: To explore treatment effects of hydroxyethyl starch 130/0.4 and sodium chloride injection(HES)on hemorrhagic shock in dogs,and to offer experimental reference of HES in treatment of hemorrhagic shock for clinic use.Methods: Twelve local dogs in Guiyang area,weighing 6.10±1.14 kg,were randomly divided to hydroxyethyl starch 130/0.4 and sodium chloride injection(HES group)and sodium chloride injection group(SC group),6 in each group.After general anesthesia with QMB(0.15ml/kg),the carotid artery was localized by colour B ultrosonic machine and opened by aseptic surgery,disinfected Y-type cannula was Penetrate and ligatured on the carotid artery for bloodletting use.Heparin sodium was used for anticoagulation.After operation of 24 h,blood was drained from the Y-type cannula in 30 min to maintain MAP at45±5 mm Hg under non-anesthesia,the volume of loss blood was 40%-50% of the total body blood volume,then the hemorrhagic shock model was made.Hydroxyethyl starch 130/0.4 and sodium chloride injection(20m L/kg body weight)and 40 m L/kg body weight of saline was administrated intravenously in HES group.otherwise,only saline(60m L/kg body weight)was administrated in SC group.The changes of blood routine parameters,especially related index of red blood cell,biochemical indexes(BUN?Crea)and electrolyte(Na+?K+?Cl-)were detected in pre-medication and post-medication of two therapeutic methods,the changes of blood pressure during and post-treatment 1 h were detected continuously,and combining clinical symptoms and rehabilitation of experimental dogs to judging the clinical effect of two treatment methods.Results: Experimental results were as follows:1.Carotid artery intubation: Success rate of carotid artery intubation was 75% in post-operation 24 hours,which could be used to make hemorrhagic shock model.2.The hemorrhagic shock model was established successfully: When the MAP was maintained at 45±5 mm Hg,visible mucosa was pale,skin was clammy and cold,pulse was weak and slow,dyspnea,urine volume was reduced,spirit was restless or indifferent,and feces were incontinence.3.Changes of blood pressure during medication: After intravenous drip infusion of HES for 15 minutes,the MAP began to rise significantly,then it increased to 61.33±6.71 mm Hg at the end of infusion.The rise of MAP was not significant and DP rose significantly at the end of infusion in SC group.Compared with SC group,MAP increased faster in HES group,the difference was significant,4.Changes of blood pressure after medication: After medication of 60 min in HES group,MAP,SP and DP were 75.17 ± 3.60?100.33±2.31 and 67.00±3.61 mm Hg,respectively.While its were 58.67±0.58 mm Hg,78.50±0.71 mm Hg,52.00±0.00 mm Hg,respectively,in SC group.The difference was extremely significant between the two groups.5.Changes of blood routine in different periods: HCT was 10.73±5.87 % in shock status,it rose to 34.00±3.25 % approaching normal value after 1h of HES administration,compared with SC group,the difference was extremely significant.Meanwhile,RBC value also increased significantly,compared with SC group,the difference was significant.PLT increased significantly after 1h of HES administration.WBC increased significantly.6.Clinical manifestations after medication: The visual mucosa was pink after 1 h of administration with HES.The mental state of dogs were good on the 2nd day,then they recovered without further treatment.In the group SC,visual mucosa was still pale,limbs were cold,breathing was short,vomiting and sleepiness.The dogs were in poor mental state and did not eat on the 2nd day after treatment,it still needed further treatment.7.Changes of biochemical and electrolyte: Parameter of biochemical and electrolyte were not anomalous.Conclusions: HES solution could rise mean arterial pressure,systolic pressure and diastolic pressure continually in shock dogs,recover HCT quickly,improve ischemic state,and stable expand blood volume.It had an effective therapeutic effect in treatment of hemorrhagic shock in dogs.
