Clinical Observation Of Statins On Atherosclerotic Acute Cerebral Infarction

Objective: The aim of this study is to evaluate the therapeutic effect of three Statins(simvastatin,pravastatin and atorvastatin)on atherosclerotic cerebral infarction(ACI).Methods: During December 2015 and December 2016,a group of 260 patients with ACI who admitted to the Neurology Department of our hospital were enrolled in our study.They were randomly divided into three groups according to the treatment strategy:simvastatin group(n = 88),pravastatin group(n = 85)and atorvastatin group(n = 87).A total of 4 ml venous blood from each patient were collected for detection before taking drugs,7 days after taking drugs,14 days after taking drugs,30 days after taking drugs.The following indicators were detected: triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterin(LDL-C),high density lipoprotein cholesterol(HDL-C)and C-reactive protein(hs-CRP);alanine aminotransferase(ALT),aspartate transaminase(AST),blood urea nitrogen(BUN)and creatinine(CREA).In order to evaluate the effect of Statins on patients' neurological function,the national institutes of health stroke scale scores(NIHSS)and Rankin scale(mRs)were evaluated.Besides,the carotid ultrasonography was performed to evaluate the effect of Statins on morphology of carotid artery,the diameter and thickness of atheromatous plaque was detected.Results:(1)The patients TG,TC and LDL-C levels of three groups after treatment was significantly decreased,but the HDL-C level was significantly increased.Btorvastatin had the best effect on improvement of blood lipid,the following was pravastatin and simvastatin.(2)hs-CRP level was decreased after taking Statins.Among the three Statins,atorvastatin showed the effect earlier than the other Statins.Person regression analysis showed that there were no significant relationship between hs-CRP and blood lipid level.(3)Both the NIHSS and mRs scores were decreased because of Statins drugs,however,atorvastatin significantly decreased the two scores.(4)The carotid plaque became smaller after taking of Statins,but atorvastatin showed significant effect.(5)Patients' renal function and FBG did not show abnormal.There were no rhabdomyolysis,intracranial hemorrhage and side effect of gastrointestinal tract detected during following up day.After treatment,the ALT and CREA significantly decreased in simvastatin group compared to atorvastatin group,however,all the indicators were at normal level.(6)The missing rates for simvastatin group,pravastatin group and atorvastatin group were 3.45%,3.22% and 3.33%,indicating that the patients' compliance of the three groups were relatively high.About 63.64%,56.47% and 58.62% patients in the three groups showed fully satisfied.Patients mainlyfocused on drug cost and side effect.Conclusion: All the three Statins shows good effect on ACI,especially the atorvastatin.
