Clinical Study Of Liangxue Zhixue Decoction In The Treatment Of Chronic AUB-E Prolonged Blood Heat Syndrome

Nongestational abnormal uterine bleeding(AUB)in the reproductive years is defined as blee-ding from the uterine corpus that is abnormal in duration,volume,frequency,and/or regulari-ty,and the prolonged menstrual bleeding is defined as menstrual periods lasting more than 7 days.Chronic AUB is the one that has been present for the majority of the last 6 months,which has a significant impact on sufferers' physical,social and emotional quality of life.On the other hand,the new classification system of "PALM-COEIN" causes the diagnosis and treatment of AUB to be more systematic and clear.Among them,AUB-E,which caused by endometrial disorders,can be characterized by heavy menstrual bleeding(HMB),prolonged menstruation,and intermenstrual bleeding.Currently,there's no specific method of diagnosis for AUB-E,which mainly based on the basis of ovulation menstruation to rule out other clear abnormalities after the determination.Western medicine pays more attention to the HBM of AUB-E,while studying it also involves related observations on the efficacy of menstrual length improvement,but there are few relevant drug selection and curative effects that are clearly proposed to treat the prolonged menstruation of AUB-E.Traditional Chinese Medicine(TCM)is effective in treating menstrual disorders causing by non-organic diseases(such as AUB-E and AUB-O),but there lacks a clear study Using TCM that aims to treat AUB-E related prolonged menstrual bleeding.On the other hand,due to emotional,dietary,and lifestyle issues,women often suffer from blood-heat-induced diseases.Therefore,blood-heat syndrome is common in the prolonged menstruation.The Chinese medicine master Xu RunSan always using Rhinoceros Horn and Rehmannia Decoction to treat hemorrhagic menstrual disorder with blood-heat syndrome,and receiving good efficacy.Professor Jing Yan inherited the dialectical thinking of Xu RunSan,and added and subtracted the constituents of Liangxue Zhixue Decoction,which is often effective in treating non-organic menstrual disorder.Therefore,considering the AUB-E prolonged menstruation of blood-heat syndrome as a point,analysis of Liangxue Zhixue Decoction,effcacy can be Implemented,thus to provide reference for clinical treatment.ObjectiveTo investigate the efficacy and safety of Liangxue Zhixue Decoction on the prolongation of chronic AUB-E menstrual period with blood heat syndrome,so as to provide reference for clinical treatment.Method40 cases of prolonged chronic AUB-E menstrual period with blood-heat syndrome were prospectively collected.The curative effect was observed with self-contrast control.The drug was taken on the third day of menstruation and shared for 7-10 days.The corresponding data were collected to evaluate the clinical effectiveness.(1)Through the comparison before and after treatment,compare the menstrual length values and their scores to analyze the menstrual recovery rate and menstrual length improvement.(2)By comparing the scores of TCM syndromes before and after treatment,we analyzed the improvement of TCM syndromes of blood-heat syndrome with Liangxue Zhixue Decoction,and analyzed the total curative effect with the length of menstrual period.(3)Through the analysis of the total curative effect of different ages,disease course,pregnancy status,and menstrual length before treatment,the relevant factors of efficacy were discriminated.Result(1)After two cycles of treatment in 40 patients,the total effective rate of Liangxue zhixue Decoction was 97.5%,and the cure rate was 47.5%,of which 19 cases were cured,6 cases were markedly effective,14 cases were effective,and 1 case was ineffective.(2)After 2 cycles of treatment,25 cases of 40 patients returned to normal menstrual length,and the menstrual recovery rate was 62.5%.After 3 menstrual cycles of follow-up,in 19 patients recovered,1 patient relapsed.The recurrence rate of AUB was 5.26%.(3)After 2 cycles of treatment,the average menstrual length was shortened from 11.53±1.97 days before treatment to 7.25±1.81 days(P<0.001),and the average TCM syndrome scores decreased from 16.08±2.61 points before treatment to 1.43±1.69 points(P<0.001).(4)19 patients recovered after 3 menstrual cycles were followed up,1 patient had recurrence,and the recurrence rate of AUB was 5.26%.(5)The correlation analysis suggested that age was a relevant factor for total curative effect,among which 20-30 years old patients had better curative effect.There was no significant correlation between the course of disease,pregnancy status,length of menstruation before treatment and total curative effect.Conclusion(1)The effect of Liangxue Zhixue Decoction in the treatment of patients with prolonged menstrual bleeding with blood-heat syndrome is definite,and it can significantly improve the length of patients' menstrual period and the score of TCM syndromes,so that the menstrual length can be restored and the AUB recurrence rate is low.(2)The age was a related factor for the curative effect of Liangxue Zhixue Decoction,among which the treatment effect of the 20-30 year old patient group was more obvious.
