Research On The Current Situation And Influencing Factors Of Chinese Citizens' Health Culture Literacy

ObjectiveThis paper evaluated a health-literacy measure instrument about TCM,and objectively described the differentiation between diverse dimensional health-literacy level,and the influencing factors of health-literacy about TCM.This paper also provided rational advice about improvement of citizens' health-literacy of TCM.MethodsHealth-literacy Questionnaire of TCM(HLQ-TCM)was used in the pre-survey in certain area through convenient sampling method.Testing the internal consistency,test-retest reliability through the item analysis and understanding the reliability and responsiveness of the questionnaire by expert consulting.Stratified multistage random sampled 89,095 respondents from 328 survey sites in 30 provinces.This survey investigated demography characters and the health-literacy level of TCM with effective questionnaire 87,635,effectiveness 93.2%.Database was established by epidata3,1,and the sample data was weighted according to the 2010 census data using SPSS22.0.The descriptive analysis,chi-square test,and logistics regression analysis were used to analyze the literacy level.Describing the health-literacy level of TCM by percentage,in the complex sampling module,this paper tested the demographic difference of health-literacy by chi-square and explored the main influencing factors of the literacy by logistic regression.Results(1)The average difficulty and differentiation of the survey questionnaire are 0.55 and 0.38 respectively,which can better measure citizens' level of health-literacy of TCM.The correlation coefficient of the retest reliability is 0.903(P<0.05),the internal validity Cronbach's a coefficient is 0.810,and the expert validity is 0.786,indicating that the questionnaire possesses good reliability and validity.(2)The overall level of health-literacy of TCM is 13.16%,and each of its four-dimensional literacy level present more than 20%,namely basic concept,healthy lifestyle,elementary cultural knowledge of TCM and comprehension ability about TCM information.While the literacy level about suitable methods which is 3.74%need to be improved.(3)Multivariate logistic regression shows that age,education level,occupation have influence on the level of health-literacy of TCM and its five-dimensional health-literacy.The factors affecting the overall quality of TCM health culture were regional,gender,and Age,education level,occupation,chronic disease status,negative(protection)factors:western(reference to the east,OR=0.628,P<0.05),teachers,other public institution personnel,students,farmers,workers,other companies Staff and others(reference to civil servants,teacher OR=0.890,other public institution personnel OR=0.765,student OR=0.671,farmer's OR=0.361,worker OR=0.624,other business personnel OR=0.814,other OR=0.63 8,P<0.05),chronic diseases(with OR=0.820 as reference without chronic disease,P<0.05);positive(dangerous)factors for literacy levels:female(reference to male,OR=1.094,P<0.05),Physician(referenced by civil servant,OR=1.925,P<0.05),Age(reference to 15 to 24 years old,other age groups were OR>1,P<0.05),Education level(Impossible FOR reference All other educational levels were OR>1,P<0.05).(4)In the five dimensions of literacy,there are influential factors for the level of literacy in the basic concepts of TCM are region,gender,age,education level,occupation,and chronic diseases,and there are factors influencing the quality of publicly-appropriate methods for Chinese medicine:age and education level.,Occupation,and suffering from chronic diseases have influence factors on the level of literacy in healthy lifestyles:region,gender,age,education level,occupation,and influential factors on the level of literacy in the common sense of traditional Chinese medicine culture are region,age,education level,and occupation.There are influential factors for the level of literacy in the ability to comprehend information in TCM.The factors are region,gender,age,education level and occupation.Conclusion(1)The measurement of health-literacy of TCM is scientific and reasonable by HLQ-TCM.(2)The overall level of health-literacy of TCM is high,while the gap between rural and urban region is significant.The authority should promote regional development and popularization of TCM through economic support and staff recruitment.(3)In terms of lower level about suitable method,theory construction and propaganda booklet compiling are indispensable.(4)Age,education,occupation,region,chronic disease influent people's health-literacy level,which making enormous difference between groups of people.Therefore,adopting precise propaganda methods according to demographic characters is important to improve people's health-literacy.Besides,the performance of TCM popularization works should be measured in according with health-literacy level of TCM,and we should pay more attention to process measurement and basic regular pattern exploring to strength the weak and resources allocation in further research.
