| Liver cirrhosis is a common chronic,progressive and diffuse liver disease caused by different etiologies.It is on the basis of extensive degeneration and necrosis of liver cells,diffuse hyperplasia of liver fiber tissue,formation of regenerated nodules and false lobules,;leading to damage to the normal structure of hepatic lobules and vascular anatomical structures.Ascites is one of the most common complications of late cirrhosis of the liver,and about 10%of patients eventually develop intractable ascites.Clinical treatment of ascites for cirrhosis mainly consists of diuretic drugs,albumin supplementation,ascites drainage,etc.,but none of them can solve the etiology of ascites for cirrhosis of liver.Liver transplantation is the only radical treatment method in western medicine,but it is difficult to be widely used due to limited conditions.In this case,the advantage of TCM treatment lies in improving the quality of life of patients and extending the survival period of patients.However the doctor of traditional Chinese medicine on the clinical treatment of ascites and no unified treatment,the treatment results of different subject that caused the acknowledgement of ascites syndrome classification are different,in France medicine have their own characteristics.How to better inherit the clinical experience of Chinese medicine practitioners in treating ascites is a problem that young doctors should solve at present.Objective:This study aims to collect Xu Chunjun professor make a diagnosis and give treatment of cirrhosis ascites patient's medical record,finishing its data,the application of modern information technology and data mining technology,through the analysis of the collection,conservation and Xu Chunjun professor make a diagnosis and give treatment of cirrhosis ascites related information,summarize Xu Chunjun professor make a diagnosis and give treatment of cirrhosis ascites clinical experience and academic thought,mining,summarized the treatment of cirrhosis ascites,academic characteristics and theory of provide experience and thinking for the clinical treatment of liver cirrhosis ascites in the future.Methods:This study included between January 2017 and December 2017,met inclusion exclusion criteria by professor Xu Chunjun during outpatient treatment and prescription of 60 patients,collecting a total of 115 cases of basis material.This topic all basis data by the ancient and moderm basis version V1.4 cloud platform system for data processing,to conform to the specifications of cases,the data acquisition using Microsoft office Excel entry,using the descriptive statistical methods to data(including gender,age,tongue,tongue coating,pulse condition,clinical symptoms,Chinese medicine,Chinese medicine syndromes and drug mixed to classics,etc.)of four gas frequency analysis,frequency analysis and arrangement analysis to carry on the preliminary statistics.Then using the correlation analysis,namely using support and confidence,and increasing degree of symptoms to mining,the link between the symptoms,such as drugs,find the rule of medication to sum up the experiences of professor Xu Chunjun clinical medication.Results:The diagnostic data showed that there were 47 consultations for men and 68 consultations for women.Analysis showed that age 60-69 age people most times,46 m,followed by 70-79 age group,a total of 29 people,40-49 ages and ages 50 to 59,the same total than the proportion of minimum age 30 and 39,less people least number is 80-89 age.The common clinical symptoms were ranked by frequency,followed by abdominal distension,fatigue and low urine output.In the statistics of tongue related data,the top three are tongue pale red,tongue red and tongue purple shade in order of frequency.In the statistics of tongue coating related data,the top three are white moss,thin white moss and little moss in order by frequency.In the pulse statistics,the first three are chords,veinlets and sinkers in order of frequency.All the western doctors diagnosed in this medical case are viral cirrhosis,alcoholic cirrhosis,liver occupying lesions,drug-induced liver injury,autoimmune liver disease and obstructive jaundice.According to the clinical manifestations and signs of the patients,all patients were classified into the following five types:qi deficiency,blood stasis,dampness and heat accumulation,spleen and kidney Yang deficiency,liver depression and blood stagnation,and liver and kidney Yin deficiency.Among the data analysis results,the frequency of qi deficiency and blood stasis syndrome was the highest,58 times.Secondly,there are 39 cases of dampness and heat accumulation.There were 36 cases of spleen and kidney Yang deficiency.There were 26 cases of liver depression and blood stagnation.Liver and kidney Yin deficiency syndrome appeared 25 times.TCM analysis results showed that 115 visits there drugs in 126,including a percentage in more than 50%of the ordered by frequency of ze LAN,astragalus root,Fried atractylodes,angelica,donkey-hide gelatin beads,radix paeoniae alba,radix scutellariae,wormwood,himself,three fairy,fructus lycii,rhizoma alismatis,Fried coke zizyphus jujube benevolence and grifola 14 herbs.According to the statistical results of sex taste analysis of drugs,the top three drugs ranked from high to low frequency in professor xu's clinical drug four-gas data analysis were flat and mild.In the data analysis of five flavors of drugs,the top three ranked from high to low in frequency are respectively sweet,bitter and spicy.According to the data analysis,the top three places that enter the viscera from high to low in frequency are spleen and stomach,liver and gallbladder and lung and large intestine successively.According to the correlation analysis,the most commonly used drug groups in clinical practice are astragalus membranaceus,astragalus albino,zellan,angelica sinensis,baishu and gelatin.Qi deficiency and blood stasis is more related to traditional Chinese medicine according to the frequency of the ranking of astragalus membranaceus,Fried atractylodes,angelica,donkey-hide gelatin,f,focal three fairy,medlar,grifola,semen pharbitidis,big belly skin,rhodiola,and cassia twig.The traditional Chinese medicine related to liver depression and blood stagnation was ranked by frequency as zellan,baishu,paeoniae paeoniae paeoniae,baicalensis,baicalba,baicalba,radix paeoniae alba,radix paeoniae miltiorrhizae,penaeus,patchouli,money grass,yu jin and chuanxiong.Related to the spleen and kidney Yang deficiency syndrome is relatively in Chinese traditional medicine according to the frequency of ranking order ze LAN,angelica,astragalus,donkey-hide gelatin,alisma,grifola,f,gall,wolfberry fruit,cassia twig,semen pharbitidis,big belly skin,focal three fairy,red paeony root and lateral root.The Chinese medicine related to Yin deficiency syndrome of liver and kidney was successively ranked by frequency:zellan,astragalus membranaceus,angelica angelica,fangji,gelatin,porcine glabra,artemisia annuae artemisia annuae,yiniae,morning glory,patchouli and ventral skin.Conclusions:This study through the analysis of professor Xu Chunjun treatment of cirrhosis ascites clinical basis,think that disease of liver cirrhosis ascites in liver and spleen kidney,pathogenesis of viscera function disorder,the sanjiao(triple energizer)gasification,lead to blood and dysfunction,phlegm retention,endogenous in deficiency of qi and blood,TanNing blood stasis on the basis of hair for ascites.The clinical syndrome differentiation of ascites patients is divided into five syndromes:qi deficiency,blood stasis,dampness and heat accumulation,liver depression and blood stagnation,spleen and kidney Yang deficiency and liver and kidney Yin deficiency.In the treatment of drugs to supplement qi yangxue,promote blood to remove blood stasis phlegm and shuli sanjiao.This study for Xu Chunjun professor's experience in the treatment of liver cirrhosis ascites clinical provides the objective data to support,help old experience of development of traditional Chinese medicine,as well as the clinical workers with new ideas. |
