| ObjectiveFacial paralysis is a common diseases in clinic that can develop at any age.It has the characteristics of directly affecting facial appearance,slow recovery,long course and easy to leave sequelae,which brings great psychological burden and mental pain to patients.Western medicine has certain limitations in treating facial paralysis.The aim of this study is to summarize the experience of modern medical practitioners in the treatment of facial paralysis,and to provide reference for improving the clinical recognition and treatment of facial paralysis.MethodThe philological approach is used to sort out the evolution of disease names.Using statistical methods such as frequency statistics and association rule analysis,the researcher analyzes the types of symptoms of facial paralysis and summarizes the rules of clinical diagnosis of facial paralysis.The prescriptions of famous Chinese medicine practitioners were analyzed to summarize to the clinical rule of medication for facial paralysis.The rules of correlation are used to analyze the prescriptions and acupuncture points,and to summarize the clinical experience of Chinese medicine practitioners in the use of drugs and acupuncture points.Combining with ancient literature,the researcher interprets the academic thoughts of famous Chinese medicine practitioners and analyze the key points of evidence identification and the characteristics of medicine use.Results(1)There were 23 types of TCM evidence involved in the medical cases of facial paralysis,including: wind evil in the ligament,wind-phlegm blocking the ligament,wind-cold attacking the ligament,wind-heat injuring the ligament,qi deficiency and blood stasis,liver wind moving inside the ligament,liver-depression and spleen deficiency,blood deficiency,liver-biliary damp-heat,liver-depression and qi stagnation,qi stagnation and blood stasis,ligament disharmony,liver-stomach disharmony,wind-damp entering the ligament,liver-kidney deficiency,liver-yang hyperactivity,yin deficiency,spleen-kidney yang deficiency,and shaoyang disease evidence.It involves deficiency,actual and mixed evidence,with actual and mixed evidence predominant.(2)The statistics of the evidence elements of facial paralysis medical cases revealed that there were 16 kinds of evidence elements involved,and the most common evidence elements were external wind,phlegm,cold,heat,blood stasis,and qi deficiency.(3)The commonly used prescriptions in the facial paralysis medical cases included Quan Zheng San,Yang Replenishing and Returning Five Soup,Xiao Xuming Tang,Yin Qiao San,Da Gentiana Tang,Da Bu Yin Wan,Er Chen Tang,Siwu Tang,Si Jun Zi Tang,Tao Hong Siwu Tang,Ge Gen Gui Zhi Tang,Xiao Chai Hu Tang,etc.(4)Thirty-five high-frequency drugs were counted according to the medication used in facial paralysis cases,among which,stiff silkworm,whole scorpion,licorice,antiphlogistic,white fenugreek,Chuanxiong,angelica,white peony,hooked vine,and astragalus were the most commonly used drugs in clinical treatment of peripheral facial paralysis.(5)By categorizing and analyzing the high-frequency drugs,we concluded that the types of drugs commonly used in the treatment of facial paralysis were wind-cold dispersing drugs,wind-heat dispersing drugs,heat-clearing and detoxifying drugs,yin tonifying drugs,heat-clearing and fire-removing drugs,and qi tonifying drugs.(6)Through cluster analysis of high-frequency drugs,14 pairs of commonly used drugs were found,including Tao Ren-Red Flower,Chuan Xiong-Adang Gui,Danshen-Geotian,Stipula-Whole Scorpion,Centipede-Earth Dragon,Scutellaria Baicalensis-Chai Hu,Radix Codonopsis Pilosulae-Atractylodes,Glycyrrhiza Glabra-Ginger,Chen Pi-Hanxia,Jinyinhua-Lian Qiao,Fang Feng-Thorn Mantle,Dahurica Dahurica-Dokusen,Hooked Vine Tribulus Terrestris,and Tian Ma-Chrysanthemum.(7)Factor analysis of the associated high-frequency drugs led to the following representative prescriptions: Zhanzheng San,Tonic Yang Returning Five Soup,Yin Qiao San,Da Gentiana Tang,Da Yin Wan,Er Chen Tang,Si Jun Zi Tang,and Tao Hong Si Wu Tang.(8)Through the acupuncture point statistics,the high-frequency acupuncture points were Di Cang,Bu Che,Hegu,Yang Bai,Si Bai,Cataract,Shia Guan,Ying Xiang,Zygomaticus,and Zan Zhu.(9)By clustering the high-frequency acupoints,it was found that the commonly used matching acupoints were Di Cang-Bu Che Che,Cheng Zhu-Shui Gou,Yang Bai-Si Bai,Sun-Yin Tang,Foot San Li-San Yin Jiao,Shuang Zheng-Tai Chong,Ying Xiang-Feng Chi,and Zhan Zhu-Shi Zhukong.ConclusionFacial paralysis is also known as mouth seclusion and mouth-eye distortion.In ancient texts,two conditions are common: one refers to the clinical symptoms of stroke,syncope,epilepsy,etc.;the other refers to the name of an independent disease,and the main symptoms include distortion of the corners of the mouth,incomplete eyelid closure,and shallowing of the frontal lines and nasolabial furrows,often accompanied by manifestations such as tearing in the wind,loss of taste,and auditory hypersensitivity.In addition,phlegm and dampness in the body or deficiency of qi and blood are also important causes of the disease.In the specific identification,modern doctors mostly use the combination of local syndrome identification and overall syndrome identification.The local exploration is used to clarify the lesion meridians and the four diagnoses together is used to identify the internal organs of Qi,blood deficiency and abnormalities.The treatment has the characteristics of staged treatment,left and right respective treatment,the combination of local and overall treatment.The medicinal flavors are based on wind and spasm control,wind-cold dispersing medicine,heat-clearing and detoxifying medicine,wind-heat dispersing medicine and yin tonifying medicine.When treating facial paralysis,drugs such as Bai Fu Zi,Huang Qi and Sheng Ma should be taken seriously.The external treatment for facial paralysis has outstanding characteristics and should be further explored and used in modern clinics. |
